    Our experiences in the surgical approach to the treatment of lung cancer

    It is considered that 2,200,000 new patients with lung cancer are detected annually in the world (Globcan 2020), i.e. 1,135 in the Republic of North Macedonia (Globcan 2020), of which only 59 patients were surgically treated at the thoracic and vascular surgery clinic in 2022, which is 85% of the total surgically treated in the Republic of North Macedonia. The use of oncological drugs before surgical treatment can contribute to the application of surgery in cases where it was not possible, as well as reduce the tumor so that a less extensive surgical procedure can be applied. At the Thoracic and Vascular Surgery Clinic, of 59 patients surgically treated for lung cancer, 49 (83%) were treated with open anterolateral thoracotomy, and lobectomy with lymphadenectomy was most commonly performed in 41 (49.5%). Early diagnosis of lung cancer, using computed tomography screening, would allow the use of less aggressive approaches, such as videothoracoscopic surgery, robot-assisted surgery, and the use of less extensive lung resections such as segmentectomies with lymphadenectomy.

    Lung cancer is the most common malignancy in men and in the entire human population. It is considered that 2,200,000 new patients are detected annually in the world (Globcan 2020), i.e. 1,135 in the Republic of North Macedonia (Globcan 2020), of which only 59 patients were surgically treated at the thoracic and vascular surgery clinic in 2022, which is 85% of the total surgically treated in the Republic of North Macedonia.1 Numerous surgical techniques of lung resections are at our disposal in the treatment of lung malignancy, from wedge resection, segmentectomy, lobectomy and pneumectomy, which depends on the stage of the disease, localization of the tumor, involvement of other structures, functional reserves, etc. Lobectomy with lymphadenectomy is still a standard surgical technique in the treatment of early lung cancer (stage I and II non-small cell cancer). The use of oncological drugs before surgical treatment can contribute to the application of surgery in cases where it was not possible, as well as reduce the tumor so that a less extensive surgical procedure can be applied.     Evaluation of the surgical treatment of lung cancer

    Lung cancer is the most common malignancy in men and in the entire human population. It is considered that 2,200,000 new patients are detected annually in the world (Globcan 2020), 475,000 in Europe (Globcan 2020), i.e. 1,135 in the Republic of North Macedonia (Globcan 2020). The treatment is multimodal, and several factors influence its choice. Surgical treatment is one of the modalities of treatment and its application depends on numerous factors -histological structure of the tumor, stage of the disease, satisfactory respiratory reserves, satisfactory cardiac reserves, general condition of the patient, etc. At the clinic for thoracic and vascular surgery in 2022, only 59 patients were surgically treated, which is 85% of the total surgically treated in the Republic of North Macedonia. Out of the 59 patients, 44 (74.6%) were men. The average age of the operated patients was 64.22 years, and 37 (62.7%) underwent neoadjuvant chemotherapy preoperatively to reduce the tumor and stage of the disease. Patients who were operated on were mostly in IIIA stage 16 (27.12%). The small percentage of operated patients, 7.7%, and the particularly high percentage of operated patients at an advanced stage indicates that patients are detected at an advanced stage of the disease, which is a contraindication for surgery. Introduction of screening for lung cancer using computed tomography in risk groups will significantly change the therapeutic approach to this group of patients, as well as the outcome of their treatment.

    INCARCERATED INCISIONAL HERNIA: AN UNUSUAL PRESENTATION OF LIPOSARCOMA MYXOIDES METASTATICUM

    Abdominal wall hernia is a common condition seen in the clinical practice of surgery. Soft tissue sarcomas are rare neoplasms, representing less than 1% of tumors in adults. However, recurrent liposarcoma reason for incarcerated incisional are rare and there are limited studies on this subject. We report a case of a 43-year-old female who presented with generalized abdominal pain and vomiting. She was treated for an incarcerated incisional hernia and underwent an exploratory laparotomy, which showed a multiseptated incisional hernia sac, with small bowel loops and the presence of recurrent liposarcoma in the mesentery. Desincarceration with maximal adhesiolysis and total extirpation of the recurrent liposarcoma and abdominal wall hernioplasty were performed. Contrast-enhanced CT is the first-line investigation, complete surgical excision remains the gold standard treatment for primary and even recurrent tumors. Prognosis depends on the histological type and grade. Histopathological examination revealed a liposarcoma myxoides metastaticum. The patient had surgeries for liposarcoma three and one years ago, and appropriate oncological treatment. The operative and postoperative course were uneventful. Postoperatively, she continued with oncological treatment. Follow-up after 6 months remains clean and is ongoing. Conclusion: Liposarcomas are aggressive malignant tumors with frequent recurrences.

    GIANT LEFT HEPATIC CYST WITH GASTRIC OUTLET OBSTRUCTION

    The prevalence of cystic hepatic lesions in the United States is estimated to be 15%-18% and simple hepatic cyst is the most common, found in 2.5%-18% of the population. Gastric outlet obstruction (GOO) is a medical condition where there is an obstruction at the level of the pylorus, which is the outlet of the stomach. Individuals with GOO will often have recurrent vomiting of food that has accumulated in the stomach, but cannot pass into the small intestine due to the obstruction. A 74-year-old patient was admitted to the emergency department due to diffuse abdominal pain, bloating and tenderness in the upper abdomen. In the last month, he had malaise, bloating (especially after a large meal), loss of appetite, regurgitation and a metallic taste in the mouth. Laboratory tests (WBC 12.000, CRP 10) and a CT scan of the abdomen with contrast were performed. We detected a multiple cystic formations on the right liver lobe and larger one on the left liver lobe that compresses the antro-pyloric part of the stomach and D-1 of the duodenum. We performed gastric emptying with a nasogastric tube, serological tests to exclude possible echinococcosis and tumor markers to exclude possible malignancy. Then, we performed a diagnostic gastroscopy and the findings were normal. An indication for laparoscopic intervention has been made and laparoscopic evacuation and excision-reduction of the simplex cyst was performed. The histopathological analysis revealed a simplex cyst. The patient had a normal postoperative course and was discharged home on the third postoperative day. At the examinations after 1 and 3 months, the findings were normal. Large cysts within the liver parenchyma are in contact with the vascular, biliary and digestive systems. The resulting compression may lead to inferior vena cava obstruction, Budd Chiari syndrome, obstructive jaundice, portal hypertension and Gastric Outlet Obstruction.

    MANAGEMENT OF A GIANT LIPOMA IN THE AXILLA: A CASE REPORT

    Lipomas are the most common benign mesenchymal tumors in adults.Generally,they are less than 5 cm in size but sometimes they can present as giant lipomas.Axillary region is an unusual localization for lipomas in general, especially for giant lipomas.A case of a 62-year-old male with a giant lipoma in the axillary region is presented.The tumor was located in the left axillary region beneath the pectoralis major muscle and extended all along to the scapula. It was adjacent to the axillary vessels and the brachial plexus, but did not invade the neurovascular bundle.A team of a plastic and thoracic surgeon performed the operation.The pathology report confirmed a benign lipoma weighting 454 grams and measuring 13.5x8x7 cm. Follow-up examinations (clinical examination and ultrasound) revealed no recurrence three years after the operation.The mechanism of lipoma formation is not completely clear. It includes twopotential and overlapping mechanisms in which trauma seem to have the major role.The treatment includes surgical excision or liposuction. However,excision of large lipomas can sometimes be a surgical challenge.Giant lipomas in axillary region should beremoved in order to establish the pathohistological diagnosis and to prevent possible compression of the neurovascular structures. In our opinion, surgical excision is a preferred method of treatmentbecause there is less possibility of damaging vital structures and it offers better control against local tumor recurrence.

    Pediatric Echinococcosis: diagnosis, urgent surgical intervention, and treatment – case report

    Echinococcus tapeworms cause human echinococcosis, which causes hydatid cysts, most commonly in the liver and lungs but also in other organs. Delays in symptom onset can lead to diagnostic issues, especially in children. Pediatric cases are rare and often lead to unintentional diagnoses. Living in a rural area increases risk. Imaging and serological testing are essential for diagnosis. This case is rare since it involves pediatric echinococcosis. Children rarely get echinococcosis, and symptoms are often vague; thus, diagnosis is mainly by accident. Case report: A 10-year-old female child developed symptoms and signs such as tachycardia, fever, and facial urticaria. On the first clinical examination, classic signs of an acute abdomen were present, indicating a possible abdominal emergency. Laboratory results showed significantly high levels of inflammatory markers, including CRP and WBC, indicating an active inflammatory process, most likely caused by infection or tissue injury. Imaging examinations, particularly a contrast CT scan of the abdomen, revealed two burst liver cysts in the peritoneal cavity, confirming the diagnosis of echinococcal cysts in the liver. We undertook an urgent surgical intervention to address the burst cysts and avert potential complications like peritonitis or sepsis. Post-operative treatment included constant monitoring of the patient's condition, antibiotic and antihelminthic medication to control infection, pain management, and supportive measures to help with recovery. Echinococcosis in children manifests as nonspecific symptoms, which often leads to a rare suspicion and coincidental diagnosis. Echinococcosis is often considered a possible cause of stomach discomfort. The standard treatment for echinococcosis usually involves a combination of antiparasitic medication and surgical removal of the cyst.

    High intraoperative PEEP with recruitment manoeuvres may improve perioperative outcomes. We re-examined this question by conducting a patient-level meta-analysis of three clinical trials in adult patients at increased risk for postoperative pulmonary complications who underwent non-cardiothoracic and non-neurological surgery.

    IMPACT OF AWARENESS-RAISING CAMPAIGN ON PREVENTION OF PYROTECHNIC-RELATED HAND INJURIES IN NORTH MACEDONIA DURING CHRISTMAS HOLIDAYS

    Firecrackers and other pyrotechnic materials are used worldwide to celebrate national, religious, cultural festivals and holidays. In our country the use of firecrackers is usually during the holiday season, particularly during New Year's and Christmas days. The majority of users of firecrackers are adolescents who want to make the celebration more exciting, not being aware of the harmful effects and severe injures they can get by using pyrotechnics. The injuries caused by using pyrotechnics can vary from moderate such as first or second-degree burns to severe injuries like traumatic amputation of the fingers and hand, resulting in life-long mutilation. These injuries are expected to have long-term functional and psychological sequalae. Precisely because of that, our clinic started an awareness-raising campaign on prevention of pyrotechnic-hand injuries (PPH campaign) named "Don't let the firecrackers ruin your holidays”. The aim of this article is to present the impact of our PPH campaign in awareness raising against the use of pyrotechnic explosives and prevention of severe hand injuries by educating 15-16-year-old students about the dangers of using pyrotechnics and emphasizing the complexity of the hand and its importance in everyday life. In our study we used an anonymous questionnaire in order to see what was the impact of our presentation and discussions on students during the PPH campaign. From our 5-year experience with this type of campaign, we can see that awareness campaigns gave excellent results in decreasing the number of hand injuries. Therefore, our goal is this PPH awareness–raising campaign to be organized every year for generations to come.

    SURGICAL TREATMENT OF PRIMARY LYMPHEDEMA COMPLICATIONS – A CASE REPORT

    We present a case of a 28-year-old woman with primary lymphedema of the right leg, with tumor formations on her right foot, causing functional disabilities. Tumor formations were removed and histologically confirmed as dermatofibromas. Five years later, the patient was readmitted due to foot problems, keratosis of the heel, as well as increased edema of the dorsum of the foot and lower leg. Liposuction and excision of lymphoedematous tissue from the dorsum of the foot, release of the constrictive band at the level of the anterior surface of the ankle with "Z" plasty, as well as liposuction of lymphoedematous tissue of the lower leg and thigh were performed. The postoperative course was without complications, with a light seroma at the dorsum of the foot. During the 4-year follow-up period, the patient showed a noticeable improvement, with a slow increase in the volume of the lower leg and thigh, which did not reach the preoperative dimensions. Unfortunately, lymphedematous tissue increased again on the dorsum of the foot as prior to surgery.
