Introduction: Acromioclavicular (AC) dislocation is a common injury with incidence of 3 to 4 per 100 000 in the general population. It is more common in younger athletic patients. In 1984, Rockwood modified Tossy et al and Allman classification to include types IV, V, and VI and we use that classification. The aim of our study is to evaluate surgical treatment of AC dislocation grade III or IV with Tight Rope system and functional results. Materials and Methods: On the Clinic of Traumatology, Skopje in the period of 2015 to 2018 year were treated 32 patients with type III (14 patients) or IV AC dislocation (18 patients) with female to male ratio 0.45 (f:m=10:22). Inclusion criteria were applied to all patients. Three radiographic views were used: AP view, true axillary view and stress view of both sides of the AC joint and CC ligament. Surgical technique for thight rope technique was performed. Results: Dash (Disabilities of the Arm, Shoulder and Hand) and VAS (Visual Analogue Scale) scores were evaluated preoperatively and postoperatively. The DASH questionnaire was used as an indicator of the impact of an impairment on the level and type of disability. Conclusions: The technique proved to be effective in treating acute AC dislocations (Rockwood type III or IV) with a high degree of excellent and good functional results.

Introduction: Cholecystitis is an inflammatory response of the body triggered by a number of mutually supporting biological mechanisms, which by creating and releasing inflammatory mediators - cytokines activate the innate or acquired immune system, which leads to neutralization of the harmful stimulus and initiation of the process of repair and regeneration of damaged tissue or its continuation as a long-term chronic process with simultaneous tissue destruction and reparation. Material and methods: The study was conducted at the General City Hospital (GCH) "8th September" in Skopje and the Institute of Immunology and Human Genetics in Skopje, in the period of 2020-2022. Statistical analysis of the data was performed with the statistical package SPSS for Windows 26.0. Results: The study included 165 subjects with gallbladder inflammation divided into 3 groups: mild, moderate and severe inflammation grade. Patients with mild, moderate, and severe inflammatory processes differed significantly in IgG levels (p = 0.049), IgA (p = 0.021), and IgM (p = 0.016) and insignificantly in IgE1 levels (p = 0.16). Patients with a severe inflammatory process had a higher prevalence of IL-2R and IL-8 than patients with a mild grade (p = 0.035; p = 0.26, respectively). The intensity of inflammatory process had a nonsignificant effect on the levels of TNF-alpha (p = 0.078), and a significant effect on the levels of fibrinogen (p = 0.001), with significantly higher levels of fibrinogen in the group of patients with severe inflammatory process compared to the group with mild grade (p = 0.0009). Conclusion: The intensity of inflammatory process affects the serum levels of inflammatory cytokines with presence of strong correlation between the severe form of cholecystitis and elevated serum levels of certain inflammatory cytokines.

Introduction: Hip fractures are the most common operatively treated skeletal injuries, predominantly affecting individuals over 65 years of age who often have multiple comorbidities. A global increase in hip fracture incidence has been observed, accompanied by unfavorable outcomes, including reduced life expectancy. This study aimed to analyze the inflammatory response after hip fracture and surgery in patients aged over 65 years, and its association with functional outcomes. Methods: This prospective, non-randomized study included patients with AO type 31A and 31B hip fractures admitted for surgical treatment. Eligible participants had demographic and clinical parameters recorded, including ASA scores, pharmacological therapy, and Katz Index scores. Blood samples were collected at four time points: within 10 hours post-injury, 48-60 hours post-surgery, on postoperative day seven, and day 30. Laboratory analyses included IL-1β, IL-6, and IL-10 levels. Follow-up data, including survival, mobility, pain, and quality of life (SF-12), were collected at 12 months. Results: The study included 40 patients (median age: 78 years; 60% female). By the 12-month follow-up, 32 participants survived, while 8 died within 4-9 months. Deceased patients had significantly higher serum levels of IL-1β, IL-6, and IL-10 at all time-points. IL-6 was identified as an independent predictor of reduced mobility (p=0.013) and quality of life (p=0.009). Conclusion: Elevated serum inflammatory markers were associated with poorer quality of life, mobility, and survival. These findings suggest potential for targeted rehabilitation programs and preventive strategies to improve outcomes in geriatric hip fracture patients.

Introduction: Hip fractures in the elderly are associated with high morbidity, mortality, and reduced quality of life. This study investigated the role of systemic inflammation, measured through serum levels of C-reactive protein (CRP) and tumor necrosis factor-alpha (TNF-α), in predicting outcomes following operatively treated hip fractures in patients over 65 years of age. Materials and Methods: A prospective, nonrandomized study was conducted involving 40 patients with hip fractures classified as 31A or 31B (AO classification). Blood samples were collected at four time points: within 10 hours post-injury, 48–60 hours post-surgery, on the seventh postoperative day, and on the 30th postoperative day. Serum levels of CRP and TNF-α were measured using ELISA. Functional outcomes, including quality of life (SF-12), mobility, and pain, were assessed at 12 months post-injury. Results: Elevated serum levels of CRP and TNF-α were significantly associated with reduced quality of life, mobility, and survival. Deceased patients exhibited significantly higher cytokine levels compared to survivors at all time points. Multivariate regression analysis identified CRP (p<0.0001) and TNF-α (p=0.017) as independent predictors of quality of life, while TNF-α was significantly associated with mobility (p=0.043). Conclusion: CRP and TNF-α may serve as valuable biomarkers for identifying high-risk patients and guiding postoperative care. Monitoring inflammatory markers could improve outcomes in geriatric hip fracture patients by enabling targeted interventions and rehabilitation programs.
