Item type:Publication, Sarcopenia, More Than Just Muscle Atrophy: Imaging Methods for the Assessment of Muscle Quantity and Quality(Georg Thieme Verlag KG, 2023-09); Ivanoski, SlavchoSarcopenia, a progressive reduction of muscle mass and function, is associated with adverse outcomes in the elderly. Sarcopenia and muscle atrophy are not equal processes. Low muscle strength in association with muscle quantity/quality reduction is currently the optimal method for assessing sarcopenia. Item type:Publication, PERSISTENT TRIGEMINAL ARTERY - ANATOMICAL FEATURES AND CLINICAL SIGNIFICANCE(Macedonian Association of Anatomists and Morphologists, 2023); ; ; ; Persistent trigeminal artery is the most common primitive carotid basilar anastomosis that persists in adulthood. The overall incidence of persistent trigeminal artery is between 0.2 to 0.76%. PTA are known to be associated with a wide range of pathology. The aim of this study was to describe the morphological characteristics of the PTA and to emphasize its clinical significance. We examined radiographs from patients who had CT angiography undertaken for a variety of clinical reasons, performed as a part of their medical treatment at the University Institute for Radiology in Skopje, RN Macedonia. The study population included 234 patients, 130 male and 104 females, mean age 57.8 years. In one patient we found PTA with overall incidence of 0.42%. CTA revealed a left PTA that arise from the internal carotid artery and communicate with the basilar artery between the origin of the anterior inferior cerebellar artery and the superior cerebellar artery. Although anatomically interesting, an awareness of the anatomy and variations of the brain arteries is clinically important for radiologists and surgeons for save performance of procedures, and forensic pathologists since variants may have forensic consequences. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, INSULINOMA OF THE TAIL OF THE PANCREAS – A CASE REPORT(Georgian Business Press LLC, 2023-05); ;Pavlevska Elena ;Jovanoska Todorova Biljana; Insulinoma is a rare neuroendocrine functional tumor of the pancreas of unknown etiology which manifests itself through hypoglycemic symptoms which resolve by administering glycose. Common autonomic symptoms of insulinoma include diaphroresis, tremor, and palpitations, whereas neuroglycopenenic symptoms include confusion, behavioural changes, personality changes, visual disturbances, seizure, and coma. In most cases, these are benign solitary tumors of the pancreas, and in 5% of the cases they are associated with MEN1 syndrome. A characteristic of the diagnosis is the presence of hypoglycemia, and increased levels of C-peptide and insulin. Further radiological verification (non-invasive imaging procedures: computed tomography and magnetic resonance imaging; and invasive modalities, such as endoscopic ultrasonography and arterial stimulation venous sampling) of the tumor are required as well as its surgical extraction. We present a case of a middle-aged male with history of recurrent hypoglycemic episodes with vertigo, sweating, tremors, anxiety, fatigue, and loss of consciousness, all of which resolved after eating food. The diagnoses were confirmed after we performed non-invasive imaging procedure, such as Computed Tomography and Magnetic Resonance Imaging. The patient underwent successful resection of the tumor, and his symptoms showed complete resolution. Despite the low incidence of these tumors, they should be suspected, in cases where the patient presents with repetitive hypoglycemic episodes, with symptoms, which resolve after eating a meal. Timely diagnosis and adequate treatment in most cases equals to complete withdrawal of symptoms. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, NUTRACKER SYNDROME(Macedonian Association of Anatomists, 2021-07-26) ;Peneva, Elena; ;Lapevska Maksudova, Darinka ;Ilievski, MitkoIn the nutcracker anatomy, the aortomesenteric angle is more acute, which leads to a compression of the left renal vein between the aorta and the superior mesenteric artery. As a result, there is an impended outflow of the left renal vein in the inferior vena cava. This is termed as nutcracker syndrome. Complex of clinical symptoms exists in the nutcracker syndrome (NS). Nutcracker phenomenon (NP) is the term used to describe an anatomic finding without any clinical symptoms. The aim of this case study was to show the use of a multiphase computed tomography as a diagnostic method of choice in dealing with cases of nutcracker anatomy in patients. We present four cases of patients who underwent computed tomography of the abdomen. Two of the cases were with anterior nutcracker syndrome, one case was with posterior nutcracker syndrome and one case with nutcracker phenomenon. Establishing of the exact diagnosis in these patients is difficult, because of the rarity and variety in symptoms. Existence of the clinical symptoms distinguishes whether the patient has NS or NP. Multiphase CT scan of the abdomen with additional reconstructions in maximum intensity projection (MIP), volume rendering (VR) and multiplanar reconstruction (MPR) as an imaging method is the method of choice in the diagnosis of NS and NP. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Anomalous origin of the vertebral artery(Macedonian Association of Anatomists and Morphologists, 2010); ; ;Aliji, Vjolca; The posterior circulation of the brain consists of the vertebrobasilar system and its branches. Vertebral arteries are responsible for about 30% of brain blood supply. The aim of this study was to examine the origin of the vertebral artery and to present the anatomic variations of the vertebral artery origin as well as the percentage of their appearance in the population. Material and methods: The origin of the vertebral artery was investigated in 30 patients; a total of 60 vertebral arteries were examined with CT angiography and DSA by Seldinger technique. Results: Abnormal origin of the vertebral artery was found in one (3.3%) patient. Vertebral artery originated directly from the aortic arch, as a second branch between left subclavian artery and left common carotid artery. Conclusion: Real abnormality of the origin of the vertebral artery is rare; in our series it was found in 3.3% of the cases. The described variations of the vertebral artery origin are not only interesting from the anatomical point of view, but they also find clinical applications in the surgery and diagnostic and therapeutic radiology. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, SOLID PSEUDOPAPILLARY TUMOR OF PANCREAS WITH ANOMALY OF HEPATIC PORTAL VEIN IN YOUNG FEMALE PATIENT: CASE REPORT(Македонско лекарско друштво = Macedonian medical association, 2018); ;Zoran Karadzov; ; Solid pseudopapillary tumors are rare tumors of the pancreas, with incidence of about 3% of all pancreatic neoplasms. They are usually found in young females, with nonspecific symptoms (abdominal discomfort, mild abdominal pain, palpable abdominal mass). One third of cases are asymptomatic and diagnosed incidentally. These tumors have a low malignant potential and favorable prognosis after surgical treatment. We present a 17-year-old female patient with abdominal pain during the last three months. Radiological investigations including US, EUS, CT revealed well-defined tumor of the pancreatic head with mixed, solid and cystic pattern. Surgery was recommended, during which an anatomical variation of the hepatic portal vein in the close vicinity of the pancreatic tumor was found. Surgical resection of the tumor was performed; pathohistological specimen defined it as a solid pseudopapillary tumor of pancreas. Diagnosis of solid pancreatic pseudopapillary tumors might be difficult without histopathological evidence, however surgery should be recommended as a treatment of choice and definitive therapy.
