Item type:Publication, LESSON LEARNED AFTER THE FIRST WAVE OF THE COVID-19 PANDEMIC IN THE REPUBLIC OF NORTHERN MACEDONIA(Prohealth Ljubljana, Slovenija, 2020-12-15)What we have known so far is that crises do not affect everyone equally and that they mostly affect the most vulnerable categories (people who use drugs, certain ethnic communities, sex workers, etc.). What we learned after the first wave of the COVID-19 crisis is described in this article. Online counseling in youth counseling centers for prevention of psychoactive substance use was not equally applicable in all settings, such as the Roma community for example. In the youth counseling centers for prevention of substance use, the number of visits by young people with “covid induced / triggered anxiety” has increased, as well as young people who coping the anxiety with alcohol and others substances. Problematic use of the Internet has increased. The flexibility of drug treatment and care programs has reduced the drop-out rate of people receiving opioid agonist treatment (OAT). Access to programs and decentralized work model across the country have also reduced treatment dropouts. The presence of all staff working with drug users at the services most of the time, the admissions that were realized immediately without waiting lists and postponed appointments and in any circumstances (in quarantines - hotels, hospitals, etc.) as wall as reduced drop-outs have prevented overdoses and suicides of opioid users in the country. The drug problem has not disappeared, it may have changed but not disappeared. The drugs continued to travel. Online drugs were available. New drug victims continued to seek help. Conclusion:We do not have the “privilege” to waive the provision of treatment and care, without delay, for all our patients seeking help no matter where they are, at home, in quarantine or coming to the services. Flexibility and pragmatism, creativity and innovation are needed to provide accessible drug treatment services for all but also the most vulnerable categories that will be specific and created for them to reduce the consequences during the KOVID-19 pandemic. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, IMPACT OF COVID-19 ON TREATMENT AND CARE OF PEOPLE THAT USE DRUGS IN REPUBLIC OF NORTH MACEDONIA(2021-04-10)The CОVID-19 pandemic has affected the entire population and overall living, but this impact is strongest on marginalized groups. Experience to date has taught us that all restrictions in every sense (financial, availability of treatment and etc.), although affecting all, are particularly strong on marginalized groups. This study discusses the impact of COVID-19 on drug users in the Republic of North Macedonia. Patient conditions were analyzed in 11 OST’s in society, 2 OST’s in Prison, 2 harm reduction programs of which one for sex workers, and 2 prevention programs for the period January-June 2020. During this period, several changes occurred that reflected on the treatment and care of drug users. The centralized treatment with buprenorphine from the Clinic for Urgent Medicine and Toxicology in the capital was transferred to OST’s throughout the country. Patients from some closed cities and municipalities receive treatment through psychiatrists at their local hospital. These two things have proven to be good practices but training of the staff is needed. Unfortunately, there have been such cases, where transportation to regional centers has been difficult due to the existing restrictive measures, so number of patients in some OST’s has decreased. In some OST’s the number of patients remained unchanged, but in most centers, the number of admissions has increased for several reasons: returned patients from other countries, difficult supply with drugs regarding restrictive measures, continuation of treatment after being released from prison, decentralization of buprenorphine treatment in regional centers. Psychosocial interventions have been drastically reduced, but pharmacotherapy has taken place without interruption in the OST’s. Staff was fully present at work most of the time. There were no cases of overdose, and suicides in the OST’s. No one was diagnosed with Covid-19, but patients were not tested for detection of possible asymptomatic cases. New admitted patients received therapy daily or 2-3 times a week and other patients received take home therapy for one week. Visits, examinations and delivery of therapy to the people who were quarantined were made by the staff from OST’s. In some cases the organization "Red Cross" delivered the therapy. The country's largest prison also had no case of COVID thanks to isolation measures and regular monitoring of the condition of newcomers. Treatment with OAT in prisons continued as before the COVID -19 period, nor overdose or suicide. Harm Reduction centers were closed resulting in decreased number of served people by outreach work that continued. Harm reduction program for sex workers does not show a slight increase. Counseling centers for young people were closed but counseling was provided on line, resulting in decreased number of served people. At the end of May they have been open and face-to-face counseling together with on line counseling continue. From the results it can be concluded that the COVID-19 pandemic has an impact on drug-related conditions and the treatment and care of drug users. While some changes have proven to be good practices, others can have repercussions for the target group and the community as a whole. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Home care and the effects of Hypericum tetrapterum oil extract in the treatment of chronic wounds during the COVID-19 pandemic(Македонска академија на науките и уметностите, Одделение за медицински науки = Macedonian Academy of Sciences and Arts, Section of Medical Sciences/ Sciendo, 2022-11-20) ;Markoska Elizabeta; ;Pechijareva-Sadikarijo Iskra ;Krstevska-Blazhevska SvetlanaThe COVID-19 pandemic and the need for social distancing brought about sudden changes in the health system and treatment strategies. Patients with chronic wounds were affected by these changes and had limited access to professional treatment in hospitals. They were at a higher risk of infection with COVID-19 due to comorbidities and advanced age. The aim of the study was to develop an appropriate protocol for the in-home treatment of chronic wounds due to the COVID-19 pandemic when access to hospitals is limited and the risk of infection for these patients is high. In our case, Hypericum tetrapterum oil extract was applied for four months on a volunteer, a 78-year-old male patient with a chronic wound, additionally infected with Pseudomonas aeruginosa and comorbidities. His healing status was monitored by measuring the wound size and microbiological analysis at certain intervals. The scab of wound DPHR2 (right lower leg chronic wound 2), with its diameters of d1 (40 mm) and d2 (20 mm), fell off after 22 days of the first Hypericum tetrapterum oil extract application. The scab of wound DPHR1 (right lower leg chronic wound 1), with its diameters of d1 (74 mm) and d2 (35 mm), fell off after two and a half months of treatment with Hypericum tetrapterum oil extract. The results of our study indicated that Hypericum tetrapterum oil extract has a significant wound-healing potential and might be used as traditional medicine in the treatment of chronic wounds. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Acute severe poisoning with disinfectant in senior aged patient-case report and overview of literature considering age influence on treatment decision in alcohol-based intoxications(2021-09); ; ; ;Afrodita Berat-HuseiniKiril NaumoskiWe present our experiences in the first case of severe suicidal poisoning with 70% ethanol-disinfectant in North Macedonia, in an elderly patient with immunocompromising comorbidities. A 66-year-old unconscious woman was admitted at our clinic, with a history of seropositive rheumatoid arthritis treated with methotrexate. She was in a coma, without signs of serotonin syndrome, recurrent episodes of cardio-respiratory insufficiency under supportive treatment without invasive ventilation, metabolic acidosis, increased D-dimer 3254 ng/mL. The toxicology screening confirmed low benzodiazepines levels and alcoholaemia of 526 mg/dL (5.26 g/L), due to ingestion of 70% ethanol. Considering the decreased biotransformation in the elderly, immunocompromising comorbidities, reports of fatal outcome in poisoned elderly patients with disinfectants under standard fluids supportive protocol, haemodialysis was initiated, with registered associated hypercoagulability which resulted in complete stabilization after 48 h of admission. Treatment protocols of poisoning with ethanol-based disinfectant in the elderly should consider timely performing haemodialysis at lower alcoholaemia levels than recommended. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Pandemic with COVID-19 and Families with Children with Chronic Respiratory Diseases(Macedonian Academy of Sciences and Arts / De Gruyter, 2020-10-02); ; ; ;Boskovska, KaterinaCOVID-19 pandemic has provoked a huge change in daily functioning in millions of people worldwide. The epidemiological measures in prevention of possible infection have increased the possible risks on the mental and physical health. We have conducted a survey in order to investigate the needs and challenges of families with children with chronic respiratory diseases. In this order, we have created a questionnaire with general information about the family, general information about the child with chronic respiratory disease, overall physical and mental health before and during the pandemic, needs and mental health condition of the parents/caregivers. This survey showed that this group of families of children with respiratory disorders have suffered financially in significant way and has changed the way they perform professional and educational patterns due to the pandemic. Most of the children were stable in their physical health, but their mental health has deteriorated. This is probably due to the regular contact with the medical staff, but not with the mental health professionals. This group of children has a significant reduction in their physical activity and increase the hours in front of TV screens. The further effect on the physical and mental health is to be investigated.
