Item type:Publication, Percutaneous Nephrostomy in the Treatment of Hydronephrosis in Renal Transplant Patients - Case Report(Macedonian Academy of Sciences and Arts/Walter de Gruyter GmbH, 2022-11-01); ; ; ; Percutaneous nephrostomy is a first-line minimal invasive treatment option for ureteral obstruction following kidney transplantation, with high effectiveness and a low complication rate. Percutaneous nephrostomy might be used as a temporary salvage therapy, providing acute decompression of the kidney collecting system and preventing graft loss. It can also function as a permanent and sometimes only possible option in transplant patients with frequent recurrences of ureteral stenosis who either fail an open surgical reconstruction or who are not good candidates for these procedures. We present two patients with acute decline in urine output after renal transplantation with radiologically verified hydroureteronephrosis of the transplanted kidney (graft) caused by stenosis of distal ureter. In both cases, nephrostomy was placed within 48 hours as a temporary salvage treatment that ameliorates renal function and prevents graft loss. The permanent nephrostomy was the only possible solution for the preservation of the graft's function in the first case because of the recurrences of ureteral stenosis after several percutaneous interventions and open-surgery ureteral reconstruction. A few episodes of nephrostomy tube-related infections were resolved with antibiotics in the first case. The second case was treated with open ureteroneocystostomy with resection of stenotic segment and reinsertion of the ureter into the bladder (ureterocystoneostomy) because of the length of the involved ureteral segment. Item type:Publication, RENAL LYMPHANGIECTASIA- AN UNUSUAL MIMICKER OF RENAL HYDRONEPHROSIS(Macedonian Association of Anatomists and Morphologists, 2021); ; Renal lymphangiectasia is a rare malformation of the renal lymphatics. It occurs in both children and adults and can be unilateral or bilateral with no gender predilection. It is a condition characterized by different degrees of dilatation of the lymphatic ducts. Because of the rarity, it can be easily misdiagnosed for other cystic masses, most commonly peripelvic cysts, renal cysts or hydronephrosis. Usually is asymptomatic and incidental finding, but in severe cases can lead to hypertension and renal failure. We report a case of unilateral renal lymphangiectasia in adult male patient with medical history of hydronephrosis and characteristic radiologic CT findings. From imaging methods, we conducted ultrasound (US), contrast enhanced computed tomography (CT) and CT angiography, because they have an important role as diagnostic procedures to recognize renal lymphangiectasia. Kidney cystic lesions revealed on ultrasound and confirmed on CT as hypodense intrarenal multiloculated findings, as well as fluid attenuation collections, not always go in favor of hydronephrosis. Knowledge of the radiological findings associated with renal lymphangiectasia can contribute to better differentiation from other conditions and with the right diagnosis, successful management and treatment can be provided for the patient. - Some of the metrics are blocked by yourconsent settings

An Unusual Mimicker of Hydronephrosis - A Case Report(Sciendo, 2020); ; Renal lymphangiectasia is very rare benign lymphatic malformation of renal lymphatics, characterized by cystic dilatation of perirenal, peripelvic, or intrarenal lymphatic vessels. One of the rare mimickers of hyd- ronephrosis and cystic renal lesions on the imaging findings. We report a case-patient with unilateral right renal lymphangiectasia with flank pain and arterial hy- pertension. The ultrasound examination revealed right kidney hydronephorosis. The renal lymphangiectasia was identified on contrast-enhanced computed tomogra- phy scan. The patient was managed conservatively with antihypertensive drugs. The report also contains a re- view of the literature on the pathophysiology of renal lymphangiectasia, clinical presentation, imaging findings, differentials, complications and treatment.
