    CHOLANGIOCARCINOMA: CLINICAL AND LABORATORY TECHNIQUES WITH SPECIAL EMPHASIS ON IMAGING TECHNOLOGIES

    Cholangiocarcinoma is a rare and highly aggressive malignancy of the biliary epithelium, marked by considerable anatomical heterogeneity and a largely nonspecific clinical presentation, factors that frequently contribute to delayed diagnosis. The purpose of this study was to examine the demographic, clinical, laboratoryand imaging characteristics of patients with cholangiocarcinoma and to assess their relationship with imaging-based anatomical tumor classification.This retrospective study included 40 adult patients with histopathologically confirmed cholangiocarcinoma diagnosed in General Hospital “8th September” in Skopje, Republic of North Macedo-nia, between January 2022 and December 2025. Clinical presentation, hepatobiliary laboratory parameters, and imaging findings obtained using contrast-enhanced computed tomography, and most often magnetic res-onance imaging with magnetic resonance cholangiopancreatography were reviewed. Based on imaging find-ings, tumors were classified as intrahepatic, perihilar, or distal cholangiocarcinoma. The study population demonstrated a male predominance (62.5%), with a mean age at diagnosis of approximately 68 years. Perihilar cholangiocarcinoma was the most commonly identified type (55%), followed by distal (27.5%) and intrahepatic tumors (17.5%). Jaundice and pruritus occurred exclusively in patientswith extrahepatic disease, whereas all asymptomatic patients had intrahepatic tumors. Laboratory analysis revealed a marked cholestatic pattern in perihilar and distal cholangiocarcinoma, with substantially higher bilirubin, alkaline phosphatase, and gamma-glutamyl transferase levels compared with intrahepatic disease. Overall, these findings highlight the importance of anatomical tumor classification and support the integrated use of imaging(MR and MRCP), clinical assessment, and laboratory data in the diagnostic evalua-tion ofcholangiocarcinoma.

    Radiology Approach to Invasive Breast Lobular Carcinoma – Initial Experience

    It is well known that invasive lobular carcinoma (ILC) accounts for approximately 5-15 % of invasive breast cancers that remain one of the most common malignancies among women globally. ILC is distinguished from the more common invasive ductal carcinoma (IDC) by characteristics histopathology features. The aim of the study was to emphasize the need of more specified diagnostic imaging approach in order to detect breast ILC, based on our institution experience. We describe a number of 17 female cases (58±7 years) who presented: breast fullness/asymmetry – 11 patients (64.7 %), pain – 2 patients (11.7 %), and the rest 4 patients (23.5 %) were asymptomatic. Diagnostic modalities used in this study were: 2D Mammography, Ultrasound, Digital Breast Tomosynthesis (DBT), and core biopsy in guidance by Ultrasound. On 2D Mammography, visible mass was found in 8 patients (47 %), and architectural distortion was found in 4 patients (23.5 %). On DBT additional 5 patients were examined from the total number of patients, and additional lesions were found such as occult lesions and additional architectural distortion. Using DBT, occult lesion was found in 3 patients (17.6 %) and architectural distortion was found in 2 patients (11.7%). Due to these inconclusive findings, DBT was carried out to improve visualization especially in those with more glandular tissue. The occult lesions were seen in 3 patients, which showed as false negative on 2D, as well as an architectural distortion in 2 patients, additionally seen on DBT. This multimodality approach, including DBT, holds promise for more reliable detection of occult lesions and distortions especially in denser breast, and for better clinical outcomes in patients with ILC, respectively.

    Obesity rise plateaus in developed nations and accelerates in developing nations

    Global reporting of obesity is commonly based on comparisons over multiple decades1 and lacks a granular and systematic analysis of its dynamics. We used 4,050 population-based studies with measured height and weight data on 232 million participants to assess the worldwide dynamics of obesity from 1980 to 2024. The rise in obesity decelerated in school-aged children and adolescents throughout the 1990s in many high-income countries, and subsequently plateaued in most at age-standardized prevalences spanning 20 percentage points, from 3–4% for girls in Japan, Denmark and France to 23% for boys in the USA. There were indications of a small decline in obesity in children and adolescents in some high-income western countries (for example, Italy, Portugal and France) since the 2000s. Similar trends were seen in some countries in Central and Eastern Europe. In adults, the rise in obesity slowed down in high-income western countries about a decade after children, followed by a plateau or possibly a small reversal of the rise in some countries (for example, Spain). In most low-income and middle-income countries, the annual absolute change in prevalence has remained stable or increased over time, even though prevalence has surpassed that of high-income countries. These highly varied dynamics suggest that the social, economic and technological trends that influence the availability, affordability and use of different foods may have helped control the rise in obesity in high-income countries, but require policy interventions in low-income and middle-income countries.
