    Clinical Futures and Analysys Of Survival in Sample of Patients Infected with SARS-COV-2 in the Spesialised Hospital for Geriatric and Palliative Medicine ”November 13”-Skopje

    BACKGROUND: New worldwide intensive studies of a new virus called severe acute respiratory syndrome coronavirus-2 (SARS-COV-2) have shown that in its clinical manifestations, the virus has an extremely different expression in different population groups, with age being found to be one of the most common and significant variables. AIM: The objective of this study is to categorize the difference between clinical and laboratory parameters of a sample of patients infected with SARS-COV-2 in the Specialized Hospital for Geriatric and Palliative Medicine “November 13” – Skopje, between survived and deceased patients, impact on the number and severity of comorbidities on the severity of the clinical picture and the survival rate. MATERIALS AND METHODS: In our study, we analyzed data from a sample of 113 patients hospitalized in our institution. The study is cross-sectional and observational, and in the methodology, we analyzed demographic data by gender and age groups, analysis of comorbidities, functional and nutritional status of patients, and risk factors for mortality and survival rate. For this purpose, we used several geriatric scores: Cumulative Illness Rating Scale scale–Geriatric (CIRS-G), degree of functional ability (Bartel), and the Geriatric Nutritional Index (GNRI) score. RESULTS: The deceased patients had a significantly higher CIRS-G score, while no significant difference in functional (Bartel) and GNRI scores was found. Multivariate regression analysis showed that lymphocytopenia and low saturation were high-risk factors for death in the geriatric population. CONCLUSION: Providing hospital-level care for the elderly with SARS-COV-2 contributes to a lower mortality rate.

    Amblyopia Treatment

    Purpose: to compare patching and atropine as treatments for moderate amblyopia in children 3 to < 7 years old Material and methods: prospective study of two groups of patients treated with patching or atropine (each group of 30 patients). The patients were from 3 to 7 years old and the visual acuity was measured with E test. The groups consisted of patients with strabismic and /or anisometropic amblyopia. The visual acuity of the amblyopic eye was -20/40 to 20/100. The visual acuity of sound eye was ->- 20/40. Inter eye acuity difference >- 3 lines. The visit schedule for the first 6 months was on 5 weeks, 17 weeks, 26 weeks, and afterwards on every 6 months. Results: Patient characteristics: 47% female. Cause of amblyopia: strabismus 38%, anisometropia 37% and combine –mechanism 24%. 26% received previous treatment for amblyopia. The other results of the study will be discussed further in the paper. Conclusions: both patching and atropine are effective treatments for moderate amblyopia in children 3 to 7 years of age. Patching produces more rapid improvement and possibly slightly better outcome. Atropine has easier administration. Both treatments were well tolerated.

    Optic disc drusen

    Purpose: We analyze and discuss the way how we manage the problems in differential diagnosis emerging from the occurence of optica disc druses with the advent of the new imaging technique of computerized tomography, ultrasonography and fluorescein angiography. Patients and metohods: We present three patients with optic disc drusen. The first patient is 48 years old female presenting with episodes of transient amaurosis but with enough fundus findings to set the suspicions of optic disk drusen. The other two cases are pediatric patients who were hospitalized for arthritis and fever of unknown origin, and in which routine fundoscopic examination reveled unilateral oedema of the optic disc. Fluorescein angiography, CT scan and B ultrasonography was done in all the the above patients. Results and conclusion: Deep sited optic disk drusen are often a problem in the differential diagnosis of the optic disc edema. THe newest imaging techniques of CT-scan and B-ultrasonography are valuable tools in solving the problem.

    The Most Frequent Complications in Anterior Uveitis

    Aims: To access the frequently and the complications in anterior uveitis. Methods: The retrospective study of 120 patients with uveitis anterior who were treated in the out patient clinic or in the hospital in the period in the period of two years. Results: Within the group of 120 examined patients with uveitis anterior, in more than half of the patients 65 (54,2%) complications were encountered; 52 (43,3%) had complication on one eye; 13 (10,8%) had complication. There were patients with more than one complication; therefore the total number of patients with compaction is less than the total number of compactions. The most frequent complications are: 1. cataract - 20 (17%) patients had cataract in uveitis anterior chronic or steroid induced cataract the 15 (14%) required cataract extraction; 2. cystoid macular edema-was present in6/15(40%) patients after cataract surgery and 2/5(40%) patients with cataract still in situ. 3. secondary glaucoma -was present in 10(12%) patients, 2 patients with uveitis anterior and 8 patients with uveitis chronic .Surgery for glaucoma was required in4/10(40%) patients, filtering in 3 patients and laser iridectomy (1 patient). 4. persisting posterior synechiae-36(30%) patients had persisting posterior synechiae. 5. corneal opacitis-7(4%) patients had corneal opacities. Conclusion: Cataract is the most frequent complication of uveitis anterior, and it is a case of visual impairment. To preserve the visual function there is a need of proper treatment of uveitis anterior and early cataract extraction.

    Diplopia and binocular vision disorders

    Aim: To answer the question, is it diplopia neurologic or is it strabismic disorder. Diplopia occurs when the eyes are not aligned. Material and method: The patients examined at the Strabology department in the University Eye Clinic, Skopje from January 2003 until December 2006.Resultats.:1.Conditions which appear neurologic but are secondary to a strabismus sensory disorders. Motility disorders esotropia, exotropia , vertical deviation, after pterygium surgery, consecutive post operative exotropia, blow out fracture, thyroid eye disease 2.conditions which appear strabismic but are secondary to a neurological disorder a.) sensory disorder b).motility disorder esotropia, exotropia, vertical deviation, exodeviation and vision loss, myasthenia gravis, skew, partial III rt nerve palsy All above will be present with pictures, fundus photography, Hess chart... Conclusion: strabismus can be a presenting sign of neurologic disease.
