Item type:Publication, Subjective Quality of Life of Women in the Perinatal Period: A Post Covid-19 Pandemic Exploration in North Macedonia(Македонска академија на науките и уметностите, Одделение за медицински науки = Macedonian Academy of Sciences and Arts, Section of Medical Sciences/Walter de Gruyter GmbH, 2023-03-01); ; ;Milutinovikj, Milosh; Subjective quality of life could be considered one of the indicators of health behavior and wellbeing of women in the perinatal period. Accordingly, the aim of this paper was to examine how women in perinatal period perceive quality of life in various domains. Its relationship to age, number of pregnancies, course of pregnancies, and method of delivery and experience with Covid-19 pandemic was investigated, as well. Our sample consisted of 366 pregnant women in any period of pregnancy who came in for regular outpatient examinations and control, those who were hospitalized due to pathological pregnancy or due to the need for intensive care, as well as women in their postnatal period, one year after delivery, who were seeking professional advice from a gynecologist. The majority were aged 20 to 30 years (53.8%). The findings showed that assessed domains of subjective quality of life were related to a variety of experiences with the Covid-19 pandemic. OBJECTIVE This study aimed to evaluate maternal and perinatal outcomes in high- and low-risk pregnancies complicated by severe acute respiratory syndrome coronavirus 2 infection. STUDY DESIGN This was a multinational retrospective cohort study involving women with laboratory-confirmed severe acute respiratory syndrome coronavirus 2 infection from 76 centers from 25 countries in Europe, the United States, South America, Asia, and Australia from April 4, 2020, to October 28, 2020. The primary outcome was a composite measure of maternal mortality and morbidity, including admission to the intensive care unit, use of mechanical ventilation, or death. The secondary outcome was a composite measure of adverse perinatal outcome, including miscarriage, fetal loss, neonatal and perinatal death, and admission to the neonatal intensive care unit. All outcomes were assessed in high- and low-risk pregnancies. Pregnancies were considered high risk in case of either preexisting chronic medical conditions in pregnancy or obstetrical disorders occurring in pregnancy. The Fisher exact test and logistic regression analysis were used to analyze the data. RESULTS A total of 887 singleton pregnancies who tested positive for severe acute respiratory syndrome coronavirus 2 infection using reverse transcription-polymerase chain reaction of nasal and pharyngeal swab specimens were included in the study. The risk of composite adverse maternal outcomes was higher in high-risk pregnancies than in low-risk pregnancies (odds ratio, 1.52; 95% confidence interval, 1.03–2.24; P=.035). In addition, women carrying high-risk pregnancies were at higher risk of hospital admission (odds ratio, 1.48; 95% confidence interval, 1.07–2.04; P=.002), presence of severe respiratory symptoms (odds ratio, 2.13; 95% confidence interval, 0.41–3.21; P=.001), admission to the intensive care unit (odds ratio, 2.63; 95% confidence interval, 1.42–4.88), and invasive mechanical ventilation (odds ratio, 2.65; 95% confidence interval, 1.19–5.94; P=.002). When exploring perinatal outcomes, high-risk pregnancies were at high risk of adverse perinatal outcomes (odds ratio, 1.78; 95% confidence interval, 0.15–2.72; P=.009). However, such association was mainly because of the higher incidence of miscarriage in high-risk pregnancies compared with that in low-risk pregnancies (5.3% vs 1.6%, P=.008); furthermore, there was no difference in other explored outcomes between the 2 study groups. At logistic regression analysis, maternal age (odds ratio, 1.12; 95% confidence interval, 1.02–1.22; P=.023) and high-risk pregnancy (odds ratio, 4.21; 95% confidence interval, 3.90–5.11; P<.001) were independently associated with adverse maternal outcomes. CONCLUSION High-risk pregnancies complicated by severe acute respiratory syndrome coronavirus 2 infection were at higher risk of adverse maternal outcomes than low-risk pregnancies complicated by severe acute respiratory syndrome coronavirus 2 infection. - Some of the metrics are blocked by yourconsent settings

Item type:Publication, Bilateral Serous Retinal Detachment in Preeclampsia: Case Report(Македонско лекарско друштво / Walter de Gruyter GmbH, 2017-06-01); ;Kjaeva Nivicka, Jana; ; Introduction. Preeclampsia presents a medical condition in pregnancy that is manifested with increased blood pressure and protein urine. Ocular involvement is rare. Retinal detachment in preeclampsia is a rare complication; it only occurs in 1-2% of severe preeclampsia but in 10% of those with eclamptic seizures. Case report. A pregnant patient G1P0 visited the outpatient clinic of the University Clinic for Ophthalmology complaining on visual disturbances. The chief complaint was blurred vision and headaches. She was in 31 week of gestation and complained that she had increased blood pressure over the last month. After initial assessment she was suspected of central serous retinal ablation (CSCR). Ocular ultrasound and posterior segment OCT (optical coherence tomography) confirmed the diagnosis.Treatment and outcome. Obstetric examination confirmed high blood (TA180/130) pressure with dipstick urine showing(+++). Unfortunately, the ultrasound showed an eutrophic pregnancy in 31 g.w. with fetus mortus inutero. The patient was administered to the intensive care unit. She had an ophthalmologic check-upat 2 weeks and one month post-partum that showed regression and visual acuity was getting better. The final check-up after 6 months revealed that retina was in place with no subretinal substantial fluid and no macular edema. Conclusion. Serous retinal detachment is a rare complication of preeclampsia. In most case it resolves spontaneously few weeks post delivery. - Some of the metrics are blocked by yourconsent settings

Item type:Publication, ROBSON CLASSIFICATION OF CESAREAN SECTION IN NORTH MACEDONIA - CURRENT TRENDS(Македонско лекарско друштво = Macedonian medical association, 2021); ; ; ; Dalipi, AdelinaAbstract Introduction. Over the last few decades, the global cesarean section rate has significantly increased and reached an unprecedented level. The World Health Organization (WHO) has advised that cesarean section (CS) rates should not rise above 15%. Several classify¬cation systems have been proposed to tackle the increased cesarean section epidemic. Most of the countries have adopted and started using the Robson (10 groups) classification as the best and the one that is the easiest internationally applicable CS classification. Aim. To present the Robson classification as a way to start better classification of cesarean section and hence to reduce the number of unnecessary cesarean section deliveries. Methods: This study was realized at the University Clinic for Gynecology and Obstetrics in Skopje, North Macedonia. It is a retrospective study where two years were compared. Results. The rate of cesarean sections for 2017 was 38.5% and for 2019 42.6%. Categorization of deliveries according to Robson criteria showed a different rate of cesarean section for each subgroup. Discussion. The implementation of the Robson classi-fication in most countries has shown a reduction in the number of cesarean deliveries and thus a reduction in overall maternal and neonatal morbidity and mortality. The analysis has shown that group 5 had the largest number of cesarean section deliveries in both years, 2017 and 2019; these were patients with previous cesarean sections. They were followed by group 1 and 2, or pri¬mi¬para with spontaneous onset and induced delivery. Conclusion. The goal of Robson clasification is to identify the target groups that contribute most in the percentage of cesarean sections and to act on these tar-get groups through appropriate education and training. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Cystis paratubaris mesonephricus lateris sinistri; their incidence, diagnostic challenge and treatment(SHMSHM - AAMD, 2020-04); ; ; ;Zlateska, SofijaBroad ligament cystadenofibroma and paraovarian mesonephric cysts and other paraovarian masses are part of the adnexal pathology found in all age groups of females from fetuses to adult women, more commonly from 3rd to 5th decade. They all contain 10-20% of all adnexal masses. Paratubal cysts, including Hydatid cysts of Morgagni, are of paramesonephric origin, while paraovarian ones originate from Wolffian ducts. Borderline or malignant tumours occur in 2-3% of cases. Broad ligament cystadenofibroma is a rare entity with only few articles published, and scarce data concerning their incidence. Paratubal mesonephric cyst is more common than broad ligament cystadenofibroma but there is also no data on their incidence. This article presents a case of concomitant presence of paraovarian cyst and Broad ligament cystadenofibroma in a 19-year-old patient, with no history of pregnancies. The patient sought medical attention due to abdominal pain. Laboratory tests showed a reduction of haemoglobin and hematocrit values, tumour markers Ca 125 and CEA were within normal limits, bHCG was negative. Transvaginal ultrasound and color Doppler revealed cystic adnexal mass on the left side measuring 43 mm of dimension, and a small amount of fluid in Douglas pouch (Figure 1). Due to abdominal pain and reduction of haemoglobin values, laparotomy was indicated and extirpation in toto of the tumor was performed. The histopathology confirmed the benign nature i.e. paraovarian cyst and Broad ligament cystadenofibroma. The case is presented regarding to extremely rarity of Broad ligament cystadenofibroma, possible urgent conditions of paratubal mesonephric cyst, way of treatment and their diagnostic challenge. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Struma Ovarii: A Rare Ovarian Teratoma and Follicular Adenoma of Thyroid Gland(Austin Publishing Group, 2022); ; ; ; Stojanovska, MIOvarian struma is a germ cell tumor in which thyroid tissue represents more than a half of the tumor and has incidence of 0,3-1% of all ovarian tumors. Most of the patients are asymptomatic and tumor is found incidentally during ultrasound, or with nonspecific symptoms, like pain or abdominal swelling. We report a case of 49-years old woman with no symptoms and an ovarian mass, which was incidentally found during a routine ultrasound examination. On ultrasound she had a tumorous mass on the left ovary with central cystic part filled with dense fluid. Color Doppler showed resistance index RI - 0.64, which did not indicate malignant nature of the tumor, and CA-125 was slightly elevated (38,4 U/ml). The result of the scoring system ROMI was 11 ponts, i.e. low risk for ovarian carcinoma. The patient underwent hysterectomy and bilateral adnexectomy. The final histopathologic report was struma ovarii with well-differentiated neoplasm of uncertain malignant potential.
