Item type:Publication, THE MOST COMMON DISORDERS OF THE HAND ASSOCIATED WITH CARPAL TUNNEL SYNDROME IN ADULTS(Институт за јавно здравје на Република Македонија = Institute of public health of Republic of Macedonia, 2021); ; ; ;Ilir ShabaniLiljana AngeleskaCarpal tunnel syndrome (CTS) is one of the most common peripheral neuropathies caused by chronic compression of the median nerve in the area of the carpal tunnel and its etiology is multifactorial. Trigger fingers and de Quervain’s disease are common disorders of the hand related to CTS in adults.Theaim of this study was to present the most common disorders of the hand such as stenosing tenosynovitis in adults with surgically treated CTS and to evaluate their demographic data.Material and methods: A total of 116 surgically treated patients with established diagnosis of CTS (clinically and by electrophysiological examination) were included in this prospective study, which was conducted at the University Clinic for Orthopedic Diseases in Skopje. Demographic data, findings of history of the disease and clinical examination were recorded and analyzed.Results: Participants with CTS included in the study were with a mean age of 55.41±10.7 years (age range 29-75). 75% of them were female. 63.8% of participants suffered from one or more comorbid chronic diseases. On admission to hospital, disorders such as trigger fingers and de Quervain’s disease were concomitantly diagnosed in 15.51% on ipsilateral hand with CTS. All disorders were surgically treated following open carpal tunnel release, as “one stage procedure”, under local anesthesia.Conclusion: Our findings have determined concomitant existence of CTS and stenosing tenosynovitis (trigger fingers and de Quervain’s disease) on ipsilateral hand, which suggests common etiological factors. Despite bunion surgery having been performed for more than 100 years, there has yet to be a technique considered as the ?Gold Standard?. The aim of the study was to compare postoperative results of Mitchell vs. Golden methods of treating moderate hallux valgus deformity. Methods.This observational case control study included 49 patients (81 feet) who had the Mitchell distal metatarsal osteotomy performed, and 49 patients (77 feet) that had the Golden proximal metatarsal osteotomy performed. The results of treatment were evaluated using Hellal?s modification of the Bonney and McNab classification and the Hallux Metatarsophalangeal Interphalangeal Score (HMIS). The statistical analysis of the results was done, thus the values p < 0.05 were considered statistically significant. Results. Both operative procedures showed successful and statistically significant postoperative results compared to the preoperative status (p < 0.001). Comparative analysis of the results from the Mitchell and Golden procedures, according to the Hellal?s modification of Bonney and McNab classification, proved that there was a high statistically significant difference in favor of the Mitchell method (p < 0.001), whereas the comparison based on the HMIS showed no statistically significant difference (p = 0.123) between the two methods. The estimated results analysis of both procedures, based on the values of hallux valgus angle, intermetatarsal angle, sesamoid position, length of immobilization, treatment duration and complications demonstrated that there was a highly significant difference in favor of the Mitchell method (p < 0.001), whereas the value of the shortening of the first metatarsal bone indicated that the shortening was greater in the Mitchell method (p < 0.001), which goes in favor of the Golden method. Regarding the flexion of the thumb of the feet operated on, there was no statistically significant difference (p = 0.723). Conclusion. The examinations performed indicated that both methods showed good postoperative results, but applying the Mitchell method they were better.</jats:p> - Some of the metrics are blocked by yourconsent settings

Item type:Publication, THE IMPORTANCE OF FOLLOWING THE ORIGINAL PONSETI METHOD IN TREATMENT OF CLUBFOOT: THE IOWA EXPERIENCE(Македонско друштво на ортопеди и трауматолози = Macedonian Association of Orthopedics and Traumatology, 2022); ;Bogojevski, Ljubomir; ; Dr. Ignacio Vives Ponseti, an American orthopaedic surgeon with Spanish origin, was born on June the 3rd, 1914 and died at the age 95 in Iowa, USA, on October 18th 2009. He was a brilliant pediatric orhopaedic surgeon, best known for his method of nonoperative treatment of clubfoot, that has become a gold standard of clubfoot treatment. His iconic paper on clubfoot management (1963) is one of the few manuscripts in orthopaedic literature which has radically changed the practice as we know it now. The Ponseti method is easy to learn but, unfortunately easy to modify because modification negatively affects the treatment outcome. That emphasizes the need to learn and follow the exact steps explained by Dr. Ponseti. The first author of this article had the opportunity to learn this method in the place where it all started, guided by Dr. Jose Morcuende, the successor of Dr. Ponseti who continues his legacy in the brightest way possible. After return from the stay at Iowa Stead Family Children’s Hospital 16 patients and 23 feet were treated with best possible adherence to the Ponseti Method. We began using the original Ponseti method in October 2019. Since then, 16 patients, 7 bilateral, 9 unilateral (23 feet in total) have been treated and evaluated. We strove as much as possible to reproduce Ponseti’s strict casting protocol faithfully, as explained in the paper. Usually, for correction of the first three components 4 to 10 casts are necessary, changed on a weekly basis. Equinus is the last deformity to be corrected. In order to avoid prolonged casting and concomitant appearance of rocker bottom deformity, the correction of equinus is facilitated by a simple operative procedure in local anesthesia. Sixty five percent of the patients had good results, 31% had acceptable results, in one patient there was poor result. Compared to the original Ponseti paper from 1963, there is not any significant difference in the results, except the bigger percent of poor results that involves only one patient in our series. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, A SURVEY ABOUT THE ATTITUDE TOWARDS COVID-19 VACCINATION AMONG THE STUDENTS AT THE FACULTY OF MEDICINE – SKOPJE, REPUBLIC OF NORTH MACEDONIA(Macedonian Association of Anatomists, 2021); ; ; ; The SARS-Cov-2 virus represents the most important challenge for public health worldwide. In order to prevent the spread of the virus, vaccines from different suppliers were introduced in the Republic of North Macedonia. Bearing in mind thatvaccination is not obligatory, the present survey was designed in order to evaluate the personal attitude towards vaccination among the first and second year students atthe Medical Faculty in Skopje. In total, 427 students consented for participation in the survey. Overall, 46% out of all participants considered that vaccination should be mandatory among the healthcare workers and medical students. Most of the participants, overall 75% oftotal, expressed the willingness to receive the vaccine for the sake of public health and for their own families. Fifty-three percentof the students were willing to receive the vaccine from any manufacturer in order to contribute tothe public health, but most of them have expressed their willingness to live with the restrictive measures for an unlimited time. The overall attitude towards general vaccination was highly positive (84% of all participants).We can conclude that, most of the first and secondyear students atthe Medical Faculty, have expressed their willingness to receive the vaccine against Covid-19 - Some of the metrics are blocked by yourconsent settings
Item type:Publication, USE OF RADIOGRAPHIC CLASSIFICATION SYSTEM IN DISTRACTION OSTEOGENESIS(Македонско друштво на ортопеди и трауматолози = Macedonian Association of Orhopedics and Traumatology, 2022); ; ; ; Purpose: To evaluate the clinical outcome in patients with different radiographic features of the regenerate within the radiographic classification system judged by the bone healing index (BHI) Material and methods: The study has included 27 patients with limb lengthening using the Ilizarov method at the Clinic for Orthopaedic Surgery in Skopje from 2013 to 2020.27 segments have been analyzed using the classification system of Ru Li, Michael Saleh, Lang Yang, Les Coulton. The classification is based on the shape of the callus and on the type of feature of distraction osteogenesis. The results were classified in two groups: Results with BHI under 50 days/cm and over 50 days/cm. The results for BHI were correlated with the shape of the callus, and with the type of feature at the distraction site. Results: In 8 of 10 cases grouped in the first of five callus shape modalities, there was a more rapid bone healing than in the rest of the cases (80%).In 5 of 8 patients classified in the second type (67,5.%), the BHI was under 50 days/cm. In the other patients representing type 3,4 and 5, BHI was over 50 days/cm in more than 66,66%, Only one patient presented type 5, with BHI of 69,45. In patients with 2,6 and 9 type of feature, BHI was under 50 days/cm. In type 8 and 10, we had to change the rhythm of distraction.In one patient with type 1 of feature, a final modification of the frame was necessary. Conclusion: Use of this classification system in limb lengthening procedures can not only detect, but also predict the healing pattern of the bone and thereby allow therapeutic corrections to be made at the appropriate time in order to improve the final outcome of the treatment.
