    The impact of various factors related to the COVID-19 pandemic on the mental health of healthcare workers in the Republic of North Macedonia

    <jats:p>Since December 2019, the world has been facing an unprecedented infection caused by a new strain of coronavirus called SARS-CoV-2. The COVID-19 pandemic, in addition to affecting every aspect of daily life, further affects the already high levels of stress, anxiety and depression faced by healthcare workers (HCWs) globally. Protecting the mental health of HCWs should be recognized as an imperative during a pandemic. This paper aims to examine, by means of an anonymous survey, the attitudes and opinions of HCWs regarding the COVID-19 pandemic, as well as their mental health condition and the causes of mental disabilities. The survey encompassed HCWs (doctors, nurses/technicians and pharmacists) engaged in COVID-19 centers at the University Clinical Center „Majka Tereza“, Skopje. The results of the conducted research indicate that during the engagement in the COVID-19 centers, the HCWs faced the burnout syndrome, fear of infecting primarily family members, colleagues or negative patients, isolation etc. Most of the participants never sought psychological help, sometimes thought about how they could no longer endure the situation, and often experienced problems sleeping, but they didn’t use sleeping or calming pills. Almost half of the participants felt that they were not adequately protected in the workplace. The negative consequences of mental health are more pronounced in HCWs who are directly engaged in the treatment of patients with COVID-19. This research contributes to a better understanding of the mental health of HCWs and the need for interventions by the system in the form of education and care.     Efficacy of herbal medicines in treatment of respiratory infections

    Introduction. Herbal medicine forms the basis of modern pharmacy, and hence, it has much to offer to patients suffering from common, acute bronchitis and flu. The aim of this study was to investigate the efficacy of the herbal medicines Broncho Protect and Andrographis Extract as dietary supplements against standard medical treatment in patients with proven acute upper and/or lower respiratory tract infection. Thisprospective, double-blind, randomization study was conducted at the PHI University Clinic for Pulmonology and Allergology in Skopje. Forty patients with clinical symptoms of acute upper and/or lower respiratory tract infection were included. Patients were randomized into two groups; the first group received standard therapy without restrictions plus placebo and the second group which, in addition to standard therapy, received Bronho Protect and Andographis extract. The results of patients' self-assessment of their health status indicated a statistically significant improvement in both groups, but no difference in the final outcome. The results of the assessment of patients’ health status by the doctors indicated that in the group that received herbal medicines all patients gotbetter. Also, auscultation findings were significantly better in patients who were treated with herbal medicines. Herbal medicines are effective in treatment and prevention of respiratory infections and provide a much-needed alternative to ineffective antibiotics, whereby patients feel they are receiving beneficial therapy, thus reducing their tendency to use antibiotics.

    Azithromycin in treatment of patients with asthma and C. Pneumoniae infection

    Chronic C. pneumoniae infection has been suggested as a cause for adult onset of asthma. There are data to suggest that infectious organisms, particularly the atypical bacteria C. pneumoniae, may be involved in asthma pathogenesis. The significance of these organisms is as yet unclear. It is not known whether this organism was allowed to persist after an infection, or was present prior to the development of asthma. The purpose of this study was to determine whether anti-chlamydial treatment with azithromycin will improve asthma symptoms and lung function in asthmatic patients positive for C. pneumoniae. For this purpose, 20 patients (mean age 39.8 years) with mild asthma were treated a median of 8 weeks with azithromycin 1000 mg once weekly. All patients had C. pneumoniae infection detected by Seeplex Multiplex PCR in sputum and positive IgG titre>1:64 and IgA titre>1:16 antibodies against C. pneumoniae. Post treatment lung function, symptom score (cough, wheezing, dyspnea), morning and evening PEF values and β2-agonist use were compared with baseline values. After 8 weeks of treatment with azithromycin there was a significant reduction in symptom score (p<0.001) and a significant improvement in lung function FEV1 (p<0.001), morning and evening PEF values p<0.05 Wilcoxon matched Pairs test. We also found a reduction in β2-agonist use, but it was not statistically significant. Treatment with azithromycin significantly improved asthma symptoms and lung function, indicating that C. pneumoniae may play an important role in enhancing the inflammatory processes in the lower airways.

    Post COVID-19 Complication Presented as Billateral Lung Cavitations- Case Report

    Lung cavitation can occur as a complication in post COVID-19 infection. We present a medical case of a lung cavitation accompanied with aeroliquid formation, as a complication of COVID-19 infection. A 49 year-old man, with active, home-threated COVID-19 infection, PCR confirmed, was primary presented with cough, expectoration of white sputum, high temperature, muscle cramps, malaise.17 days after the diagnosed COVID-19 infection, the patient has requested medical care due to cough and hemoptysis. Computed tomography (CT) scan of the thorax was performed and was detected a massive right sided pneumothorax with a shift of the mediastinal structures to the left side, that underlined to a drainage treatment. The repeatedly hemoptysis along with chest pain, indicated a repetition of the CT scan of the thorax with a contrast series in which was detected a reexpansion of the right lung with an aeroliquid collection in the postero-basal segment of the right lung and a pneumatocele with a ticked wall was detected, in the superior segment of the left lower lobe. With echosonography control, a punctate sample was taken from the aeroliquid formation, with a bloody content, that was sent for microbiological analysis and proofed to be sterile. A thoracic surgeon was consulted, and was indicated surgical treatment.

    Measuring nasal volumes with acoustic rhinometry

    Acoustic rhinometry is used to objectively measure the minimal cross sectional area and volumes of nasal cavities. However, no data for healthy subjects has been reported in Macedonia. Therefore, we wanted to establish the normal range among healthy adults and to evaluate the changes after nasal decongestion.
