    FACTORS INFLUENCING THE EARLY POSTOPERATIVE QUALITY OF LIFE IN PATIENTS TREATED SURGICALLY FOR GYNECOLOGICAL MALIGNANCIES

    ABSTRACT Aim: To evaluate the influence of inherent patient characteristics on early postoperative QoL in patients treated surgically for gynecological malignancies Materials and methods: The study was designed as a prospective cohort study. Patients scheduled for surgical treatment of a gynecological malignancy at the Department of gynecological oncology at the University Clinic of Gynecology and Obstetrics in Skopje, in the period January – December 2018. Quality of life was quantified using a standardized and validated questionnaire (FACT-G) preoperatively and 1 month after surgical treatment. We explored the effect of the following variables: age, BMI, nationality, education, marital status, place of residence, employment, primary location of the neoplasm, disease stage, extent of surgical resection, comorbidities and smoking. Results: Data from 123 patients were included in the final analysis. The average age of patients at the time of recruitment was 58±10 years (range 23-79 years). Eighty-four patients (68.29%) had endometrial cancer, 22 (17.89%) patients had cervical cancer, while 17 (13.82%) patients had ovarian cancer. Postoperative FACT-G scores were significantly lower (p=0.015). A clinically relevant postoperative decrease in quality of life was identified in 51 patients (41.5%). Independent predictors of deteriorated postoperative QoL in this study were: advanced disease stage, extensive surgical resection, comorbidities and higher education. Conclusion: The results illustrate the physical, psychological, and social effects of the surgical treatment on the early postoperative QoL, thereby emphasizing the need for a comprehensive, multidisciplinary approach to the pre and postoperative care of these patients.

    Subjective Quality of Life of Women in the Perinatal Period: A Post Covid-19 Pandemic Exploration in North Macedonia

    Subjective quality of life could be considered one of the indicators of health behavior and wellbeing of women in the perinatal period. Accordingly, the aim of this paper was to examine how women in perinatal period perceive quality of life in various domains. Its relationship to age, number of pregnancies, course of pregnancies, and method of delivery and experience with Covid-19 pandemic was investigated, as well. Our sample consisted of 366 pregnant women in any period of pregnancy who came in for regular outpatient examinations and control, those who were hospitalized due to pathological pregnancy or due to the need for intensive care, as well as women in their postnatal period, one year after delivery, who were seeking professional advice from a gynecologist. The majority were aged 20 to 30 years (53.8%). The findings showed that assessed domains of subjective quality of life were related to a variety of experiences with the Covid-19 pandemic. The results are presented and discussed in detail. Implications and limitations are given, as well.

    Radical radiotherapy for squamous cell carcinoma of the larynx - comparison of three-dimensional conformal radiotherapy with cobalt-60 teletherapy

    The aim of the study was to report the results of radical radiotherapy performed by three-dimensional conformal radiotherapy (3DCRT) for squamous cell carcinoma of the larynx and to compare these data with those obtained with two-dimensional radiotherapy (2DRT) realized with cobalt-60 teletherapy i.e. telecobalt therapy (TCT). Eighty patients with previously untreated laryngeal cancer were irradiated with curative intent at the University of Radiotherapy and Oncology Clinic (UCRO) in Skopje between February 1999 and December 2008. Radical radiotherapy with the TCT unit was performed on 38 patients between February 1999 and May 2005. From June 2005 to December 2008, 42 patients were treated with a linear accelerator using 3DCRT. Complete response rates three months after completion of radiotherapy were 84.2% (32 of 38) and 92.6% (39 of 42) in the group irradiated with TCT unit and in the group treated with 3DCRT, respectively. No statistically significant differences were observed either in locoregional control (LRC) or overall survival (OS) between the patients treated with two different radiotherapy techniques. The grade of acute reactions of the skin and the larynx differed significantly between the accomplished radiotherapy techniques (Nonparametric Mann-Whitney U Test; U=577.0; Z=-2.129; p=0.012, and U=497.0; Z=-2.90; p=0.001, respectively). There were statistically significant differences observed in the grade of late effects in the skin and in the subcutaneous tissue between the radiotherapy techniques used (Nonparametric Mann-Whitney U Test; U=425.0; Z=-3.593; p=0.001 and U=637.0; Z=-1.551; p=0.035, respectively). According to the confirmed advantage of 3DCRT in terms of reduced treatment toxicity observed in our study, we consider conformal techniques being a basis in definitive radiotherapy of squamous cell carcinoma of the larynx until the new revolutionary techniques will be clinically available.

    Ankle fractures are common injuries. Since the recognition of the importance of syndesmotic injury in ankle fractures, much of the scientific work has been focused on concomitant syndesmotic injury. Despite the invention of novel devices for restoration and maintenance of the congruent syndesmosis following syndesmotic injury, the metallic syndesmotic screw is still considered to be the "gold standard". The aim of this study was to compare the clinical results in patients who retained the syndesmosis screw with those in whom the screw was removed following open reduction and internal fixation of the malleolar fracture associated with syndesmosis disruption.
