    Rhabdomyolysis and Acute Kidney Injury in a Patient with Severe Form of Covid-19 Pneumonia- A Case Report

    Introduction. The ongoing pandemic with the novel Corona virus poses unprecented challenges for the me- dical professionals worldwide. Acute kidney injury is frequently seen in patients infected with corona virus and often associated with a poor patient outcome. Rhabdo- myolysis has been recognized as one of the possible contributing mechanisms. Case. A 68-year-old man was referred to the emergen- cy department complaining of a dry cough, myalgia, general weakness with devastated energy feeling, chest pain and difficulties in breathing, symptoms he expe- rienced in the past five days. He also noticed that his urine was dark and in reduced amount. Quick antigen test for SARS CoV2 was performed, and the patient found Covid-19 positive. He was admitted at the hos- pital ward in a covid-designated unit. Laboratory findings revealed elevation of the inflammatory markers and elec- trolyte disbalance. Metabolic degradation products were markedly increased, serum urea was 44mmol/L (RF=2.7- 7.8 mmol/L) and serum creatinine 689umol/L (RF=45- 109umol/L), when deterioration of the kidney function was diagnosed. Urgent intermittent hemodialysis treatment was initiated. Patient suffered from a severe form of covid-19 pneumonia and was continuously on high flow oxygen mask. Duration of the patient hospitalization was 30 days, and thereafter, he was transferred to the reha- bilitation center for 28 days. Complete restoration of the physical motion and activity was accomplished, oxygen support was no longer needed, since he main- tained blood oxygen saturation above 95%. Renal func- tion has also been recovered with degradation products maintained within normal ranges. Conclusion. Rhabdomyolysis in covid-19 patients should be always kept in mind. Sometimes it can be an initial clinical manifestation in covid-19 patients [15], but on the other hand it can be presented as a late complication sometimes caused by the therapy itself. Multidiscipli- nary and comprehensive approach in the diagnosis, treat- ment and follow up of the patients can only guarantee timely detection and wide range of therapeutical moda- lity, leading to a better prognosis and outcome.

    The Level of Cholesterol in COPD Patients with Severe and Very Severe Stage of the Disease

    BACKGROUND: High blood cholesterol is part of metabolic syndrome and can be caused by medical conditions or bad dietary habits. AIM: The aim of the study was to investigate the prevalence of hypercholesterolemia in privies diagnosed patients with the severe and very severe stage of COPD, which аre stable. MATERIAL AND METHODS: We investigated 100 subjects, all of them smokers, with smoking status >10 years, stratified into two groups: with severe and very severe stage of the disease. It was clinical, randomized, cross-sectional study. Besides demographic parameters and functional parameters, body mass index, cholesterol, LDL, and HDL were investigated. RESULTS: In the group of patients with very severe COPD were recorded significantly higher average values of cholesterol (6.16 ± 1.5 vs.5.61 ± 1.1,p = 0.039). As independent significant factors influencing cholesterol in the group with a very severe COPD were confirmed the age of the patients (p = 0.005), LDL (p = 0.004) and HDL (p = 0.002). In the group with severe COPD, only LDL was confirmed as an independent significant factor that has an impact on cholesterol (p <0.0001). CONCLUSION: The results of our survey demonstrated a high level of blood cholesterol and LDL, and low level of blood HDL in both investigated group’s patients with COPD

    Relationship between Vitamin D, Inflammation and Lung Function In Patients with Severe Uncontrolled Asthma

    BACKGROUND:Recently epidemiological studies showed that low vitamin D is linked toairway hyperresponsiveness, decreasedlung function, poor asthma control, and steroid–resistant asthma.AIM:We investigatedtherelationshipbetween Vitamin D, inflammation with circulating IL-33 and lung function in 30 patients with severe uncontrolled asthma. MATERIALS AND METHODS:The study included 30 patients with severe uncontrolled asthma. In each of them were measured serum levels of IL-33 and Vitamin D by the ELISA method. The pulmonary function is measuredby basic spirometry parameters, FEV1. The results were statistically elaboratedaccording to the Pearson’s Correlation Tests. RESULTS:The results showed statistically insignificant correlation between Vitamin D and IL-33, and Vitamin D with FEV1 (Vit.D/IL-33; r=0.11323, p=0.551); (Vit.D/FEV1; r=-0.1005; p=0.597) Correlation between IL-33 and FEV1 is negative but statistically significant (IL-33/FEV1; r=-0.5248; p=0.003). CONCLUSION:Because there arelittle studies about the link between vitamin D and asthma, further research toclarify the mechanism how vitamin D control the activity of CD4+ T cells and the related Th2-type cytokines in the parthenogenesis of asthma.

    The effect of smoking on lung function and respiratory symptoms in population of 25-40 years who worked in state administration

    Among young people, the short-term health consequences of smoking include respiratory and non-respiratory effects, addiction to nicotine, and the associated risk of other drug use. Long-term health consequences of youth smoking are reinforced by the fact that most young people who smoke regularly continue to smoke throughout adulthood. In that aim we investigated 230 subjects from January 2012 to December 2013: 150 males and 80 females, 110 (47,8 %) non-smokers, 80 (34,78 %) smokers and 40 (13 %) ex-smokers, age between 25 and 40 years, who worked in state administration. Smoking impairs lung function: lower FEV1 values in smokers were found compared to non-smokers. Smoking increase cough and sputum production. In our survey, a significantly higher frequency of symptoms was observed in smokers compared to nonsmokers. We did not prove the influence of smoking on wheezing or breathlessness.

    Efficacy of herbal medicines in treatment of respiratory infections

    Introduction. Herbal medicine forms the basis of modern pharmacy, and hence, it has much to offer to patients suffering from common, acute bronchitis and flu. The aim of this study was to investigate the efficacy of the herbal medicines Broncho Protect and Andrographis Extract as dietary supplements against standard medical treatment in patients with proven acute upper and/or lower respiratory tract infection. Thisprospective, double-blind, randomization study was conducted at the PHI University Clinic for Pulmonology and Allergology in Skopje. Forty patients with clinical symptoms of acute upper and/or lower respiratory tract infection were included. Patients were randomized into two groups; the first group received standard therapy without restrictions plus placebo and the second group which, in addition to standard therapy, received Bronho Protect and Andographis extract. The results of patients' self-assessment of their health status indicated a statistically significant improvement in both groups, but no difference in the final outcome. The results of the assessment of patients’ health status by the doctors indicated that in the group that received herbal medicines all patients gotbetter. Also, auscultation findings were significantly better in patients who were treated with herbal medicines. Herbal medicines are effective in treatment and prevention of respiratory infections and provide a much-needed alternative to ineffective antibiotics, whereby patients feel they are receiving beneficial therapy, thus reducing their tendency to use antibiotics.
