The aim of our study was to investigate alcohol-related deaths in the Republic of North Macedonia in the period from 2007 to 2020, in order to study the influence of elevated blood alcohol levels in violent deaths. Five hundred sixty-four post-mortem blood samples from alcohol-related death cases-natural deaths and violent deaths (suicides, accidents, and homicides)-were analyzed, and the results were evaluated according to sex, age, and cause of death. Among 564 cases, traffic accidents were the leading cause of violent death (54.3% of the cases) followed by suicides (19.9% of the cases). In the examined post-mortem samples, BAC values ranged from 0.15-6.20 g/L. The average age was 45 ± 16 years for the male and 49 ± 19 years for the female group. The biggest proportion of high BAC values was found in the group of accidents specifically road traffic accidents and accidental intoxication as well as in the group of bolus deaths. The analysis of BAC in the cases of violent deaths in the Republic of North Macedonia confirmed that consumption of alcohol is strongly related to violent deaths. The data obtained from this study could raise caution and give aid in a national strategy for the prevention of alcohol-related violent deaths. - Some of the metrics are blocked by yourconsent settings

Contusions are injuries of the brain dominantly incurred by blunt-force trauma in the head (blow, impact, fall). Proper analysis of the cerebral contusions within all head injuries and data case gives us complete view of the event and enables us to choose the right direction in the investigation. The aim of this investigation was to perform a forensic medical analysis of cases with brain injury (cerebral contusions) incurred by blunt-force trauma, and to compare results with a prior extensive research done at the Institute of Forensic Medicine, Skopje. Methods. A total of 76 cases with cerebral contusions incurred by blunt-force trauma were included. The following data were analyzed: gender, age, event, skull fractures, type of fracture, localisation of contusions and toxicological analysis. The basic descriptive processing included the above information-variables taken from the written reports of the performed autopsies. The data are displayed graphically with the use of Microsoft Excel 2007. Results. Male was the dominant gender in the analyzed cases (86%), with mean age of 51 years. Traffic accidents were the main factor for these injuries (76%). Skull fractures were found in 67% of cases. The dominant cerebral lobes were the temporal and the frontal lobe, and the contrecoup contusions were found in 55% of cases. Conclusions. The experience gained during the processing and analysis of these brain injuries answers the most complicated questions, and contributes to the healthcare especially in dealing with the diagnosis and treatment of cerebral contusions. - Some of the metrics are blocked by yourconsent settings

Item type:Publication, DEVELOPMENT AND VALIDATION OF A QUANTITATIVE HS-GC/FID METHOD FOR ALCOHOL ANALYSIS IN BIOLOGICAL SAMPLES: STABILITY ASSESMENT(Romanian Society of Legal Medicine, 2022); ;Bozhinoska, Zorica ;Vujovic, Maja ;Milosavljevic, BiljanaDetermination of blood alcohol concentration is one of the most common analyses in forensic toxicology laboratory practice. With reference to this, the aim of the present work was to develop a method for quantitative analysis of ethyl alcohol in blood by head space gas chromatography, with n-propanol as an internal standard and at the same time ensuring good resolution of ethyl alcohol and acetone using one capillary column. Validation parameters comprising selectivity, specificity, linearity, accuracy and precision have indicated that the method could be applied in routine analysis of ethyl alcohol in post-mortem and ante-mortem blood samples as well as in other biological fluids, in line with the set criteria based on the revised international guidelines. In post-mortem samples there are many interfering substances due to various reasons (as a result of acidosis, produced in vitro on account of an improper storage of the samples or during the autolysis / putrefaction phase of the body). Because of this occurrence, within the validation procedure, more attention was imparted to the stability of the samples, including processed sample, freeze-thaw and long-term stability estimation. The results of the additional stability tests have shown that applying of preservative significantly improved the stability of the samples by inhibiting the in vitro ethyl alcohol production. - Some of the metrics are blocked by yourconsent settings

Item type:Publication, Pattern of benzodiazepine use among death cases in Republic of North Macedonia(Macedonian Pharmaceutical Association, 2021); ;Bozhinoska, Zorica; ; The aim of this study was to determine the pattern of benzodiazepine related deaths in Republic of North Macedonia. Retrospective survey of post-mortem toxicology data from eight-year period, obtained from Laboratory of Forensic Toxicology was performed, including cases with known gender, age, cause and manner of death and complete toxicology report. Post-mortem toxicology analyses were conducted with Fluorescence Polarization/Enzymatic Immunoassay and Biochip Array Technology method for screening of urine and blood respectively, and confirmation by GC-MS after solid phase extraction. Total of 1169 cases were investigated. Benzodiazepines were detected in 30.05% of the cases, with statistically significant difference between cases of natural and of violent cause of death, as well as between genders, but without statistically significant difference due to the age between both genders. Study revealed women were more likely to use benzodiazepines with the median age to be 52.88 and 49.87 for males and females, respectively. According to post-mortem toxicology data benzodiazepines are the most used drugs in our country. Of them, diazepam was found to be the most often used one. Data analyses have shown that benzodiazepines were often combined with alcohol, psychoactive medicines and drugs of abuse. In 46.6% of all cases with positive findings for benzodiazepines, use of drugs of abuse was detected, mostly heroine and methadone. Important note should be given to the simultaneous use of benzodiazepines with heroine and methadone, which have resulted in increased number of fatal intoxications. Further studies are needed to determine the reason for popular use of benzodiazepines among drug addicts. - Some of the metrics are blocked by yourconsent settings
