Item type:Publication, Level of nitric oxide in hypertensive patients scheduled on general anaesthesia(Macedonian Academy of Sciences and Arts, 2005-08); ;Trajkovska, T ;Soljakova, M ;Simjanovska, LIsijanovska, RIn this prospective study we have analysed the level of nitric oxide in hypertensive patients scheduled for general anaesthesia. In the study were included thirty-four patients with chronicle inflammatory disease of the middle ear who have undergone surgical treatment at the Clinic for Ear, Nose and Throat Surgery. The aim of our study was to determine the plasma level of nitric oxide (NO) and its effects on the circulatory system in hypertensive patients during the general anaesthesia maintained with inhalation of oxygen and nitrous oxide (O2/N2O) mixture. Patients were divided in two groups. During the maintenance of general anaesthesia the patients from the first group were ventilated with O2/N2O, while patients from the second group were ventilated with oxygen and air (O2/air) mixture. The other principles during the general anaesthesia were equal for both groups. For determination of the NO plasma levels we have used the enzymatic method according to Conrad et al., 1993. Our results showed that there is a statistically significant difference of NO plasma level between the two groups. The level of NO was higher in the first group (ventilated with O2/N2O) compared to the second group (ventilated with O/air). The mean arterial pressure and systemic vascular resistance were significantly decreased in the first group, as well. Our results suggest that nitrous oxide (N2O) most probably plays the role of NO donor in hypertensive patients during the maintenance of the general anaesthesia with N2O/O2 mixture. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Vascular Lipoleiomyoma of the Uterus: an Unusual Case(2016-12) ;Chibisheva, Vesna; ;Trajanova, Milka; Uterine lipoleiomyomas are a rare type of uterine myomas, which have been seldom diagnosed and are less known and still unusual. Histologically they are composed of smooth muscle cells with multiple fat cells and fibrous tissue. Their pathogenesis is still a stigma and their preoperative diagnosis is still questionable. Case report history A 39-year old woman presented at the University clinic for obstetrics and gynecology with complaints of a mild low abdominal and pelvic pain, increased frequency of vaginal bleeding and abdominal distension. Ultrasound examination showed dislocated uterus by a large semisolid tumor, located between the uterus and the urinary bladder. This mass was not well defined and had great vascularity. Pelvic computerized tomography revealed a large heterogeneous tumor located behind the bladder, measuring 8.5x7.5cm in size, making compression on the other pelvic organs. Conclusion: We should pay more attention on the preoperative imaging studies, in order to select patients who require surgery and further treatment more appropriately. - Some of the metrics are blocked by yourconsent settings

The connection between the patient and the surgeon cannot be replaced by telemedicine. For example, the surgical staff faces more serious difficulties compared to non-surgical specialists during the COVID-19 pandemic. The primary concerns include the safest solutions for protecting healthcare staff and patients and the ability to provide adequate surgical care. Additionally, the adverse effects of any surgery delays and the financial consequences complicate the picture. Therefore, patients' admission during the COVID-19 pandemic should be taken into consideration, as well as preoperative measures. The COVID-19 situation brings particular risk to patients during surgery, where preoperative morbidity and mortality rise in either asymptomatic or symptomatic COVID-19 patients. This review discusses the recent factors associated with surgical complications, mortality rates, outcomes, and experience in COVID-19 surgical patients. - Some of the metrics are blocked by yourconsent settings

Item type:Publication, Assessment of the Level of Middle Colic Artery Ligation Influence on Survival in Right-Sided Colon Cancer(Walter de Gruyter GmbH, 2025-06-01) ;Ulusoy, Cemal ;Duman, Mehmet GürayAim: This study aims to investigate whether the addition of ligation of the Middle Colic Artery in its origin to Complete Mesocolic Excision (CME) with Central Vascular Ligation (CVL) affects overall survival and lymph node retrieval. Method: Patients with right-sided colon cancer who underwent curative right or extended right hemicolectomy with CME and CVL were included. In cases with tumors located in the distal part of the ascending colon and the hepatic flexure, the middle colic artery was ligated in its origin. For the cecal and proximal localization of the tumor, the point of ligation was decided on the surgeons' decision. Results: A total number of 169 patients were operated. In 78 patients the tumor was localized in the caecum, in 70 patients in the ascending colon, and 21 in the hepatic flexure. In 59 patients, the middle colic artery was ligated in its origin. Different artery ligation sites according to tumor localization subgroup presented with statistical significance (P = 0.004). The mean number of harvested lymph nodes between the groups was presented with a difference (33.4 vs. 28.5; P = 0.037). Survival rates for cecal, ascending, and hepatic flexure cancer were as follows: 61.5%, 67.1%, and 85.7%, respectively (P = 0.22). The survival rate for all patients was 66.9% (69.5% for patients with middle colic artery ligation and 65.5% for the right branch of middle colic artery ligation, P = 0.42). Conclusion: Ligation of the middle colic artery in its origin did not improve overall survival. However, it resulted in a significantly higher number of harvested lymph nodes.
