Item type:Publication, Percutaneous Imaging-Guided versus Open Musculoskeletal Biopsy: Concepts and Controversies(Georg Thieme Verlag KG, 2020-12); ;Ivanoski, Slavcho; Bone and soft tissue tumors are a largely heterogeneous group of tumors. Biopsy of musculoskeletal (MSK) tumors is sometimes a challenging procedure. Although the open biopsy is still considered the gold standard for the biopsy of MSK lesions, core needle biopsy can replace it in most cases, with similar accuracy and a low complication rate. The biopsy should be performed in a tertiary sarcoma center where the multidisciplinary team consists of at minimum a tumor surgeon, an MSK pathologist, and an MSK radiologist who can assess all steps of the procedure. Several factors can influence the success of the biopsy including the lesion characteristics, the equipment, and the method used for the procedure. This review highlights some of the important aspects regarding the biopsy of the MSK tumors, with special attention to imaging a guided core needle biopsy and highlighting some of the recent advancements and controversies in the field. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, TALOCRURAL ARTHRODESIS USING THE METHOD OF ILIZAROV(Faculty of Physical Education, Sport and Health, Skopje, Republic of Macedonia, 2020); ; ; ; Background: Talocrural arthrodesis using the method of Ilizarov is a reasonable procedure for patients with secondary ankle degenerative changes and comorbidities, making other surgical treatment impossible. The aim of this study is to evaluate the results of talocrural arthrodesis as union rate, deviations, functional restoration and complications. Patients and Methods: The study has included 26 patients with Ilizarov talocrural compressive arthrodesis at the Clinic for Orthopaedic Surgery in Skopje from 2015 to 2019, of which 3 were with simultaneous gradual distraction. The follow up was from 22 to 64 months. The age varied from 41 to 77 years. All patients were with comorbidities and poor bone and soft tissue condition. The monitoring was performed using native radiography and ultrasonography at the compression/distraction site. Results: A complete bone fusion was achieved in all patients. The median time of wearing the external fixator was 22 weeks. In three cases with simultaneous distraction, the average bone healing index was 47 days/cm. The final maximal length discrepancy was 11 mm. In 16 patients (61.53%) sings of pin tract infections were detected. The mean number of pin site infections was 2.3/patient. No pin infection has required a change of the pin. Once the construct was removed, patients used a cast for 4-6 weeks and a walker boot for next 6-8 weeks. Conclusion: A talocrural arthrodesis performed by the Ilizarov method offers a high union rate, few deviations and a good functional restoration, often with no additional surgical procedures. In cases with numerous comorbidities and a poor limb condition, it can be a method of choice instead of an arthrodesis using internal fixation in the treatment of end-stage ankle arthritis. - Some of the metrics are blocked by yourconsent settings

Cyril and Methodius University in Skopje, 2024); ;Todorova, Teodora; ; Proposed mechanisms for extraosseous 99mTc - MDP uptake are extracellular fluid expansion, enhanced regional vascularity and permeability, and elevated tissue calcium concentration. It can be due to nonmalignant causes, such as parathyroid adenoma, vitamin D intoxication or Paget disease or can be of malignant origin. Item type:Publication, Monitoring the reduction and maintenance of periprosthetic bone tissue in cementless primary hip endoprosthesis with alendronate therapy(Scientific Foundation SPIROSKI, 2021-11-20) ;Shabani, Ilir; ; ;Popovski, NeronLoss of periprosthetic bone tissue in primary hip endoprostheses is common in clinical practice. This loss can be progressive and in extreme conditions can jeopardize the longevity of the prosthesis. In order to monitor the function of Alendronate therapy for bone maintenance, the study included 50 patients with implanted total cement-free hip endoprosthesis (TPH). The first group of 25 patients received Alendronate, calcium and vitamin D3 orally postoperatively. The second group of 25 patients were examined postoperatively without therapy. Patients were followed by radiographic and dual-energy X-ray absorptiometry (DXA) at 6 and 12 months. The study showed that in patients with TPH there was a difference in the X-ray findings as well as occurrence of osteolysis in certain Gruen zones, which was confirmed by changes in the state of bone mineral density (BMD) and bone mineral content (BMC) in the interval between 6 and 12 months using the DXA method. Alendronate therapy after TPH implantation allows reduction of periprosthetic bone mass loss, maintenance of bone mineralization and implant hardening. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Low-Grade Malignancy Glomus Tumor in a Setting of Multiple Glomus Tumors - Case Report(2019-12-15); ; ; ; Glomus tumors are rare neoplasms accounting for less than 2% of all soft tissue tumors but multiple lesions may be seen in up to 10% of the patients. Solitary glomus tumor (GT) most frequently appears as small nodule in specific locations such as subungual region or deep dermis. However, rarely these entities have been observed in extracutaneous locations such as the gastrointestinal, cardiovascular, respiratory tracts, and other visceral organs. Item type:Publication, Ultrasound-guided Biopsy of Musculoskeletal Soft-tissue Tumors: Basic Principles, Usefulness and Limitations(Sciendo, 2022-04); ;Ivanoski, Slavcho; The aim of this article is to provide a short review of the literature concerning the basic principles, usefulness and limitations of ultrasound-guided biopsy of musculoskeletal soft-tissue tumors, with particular focus on core needle biopsies. Musculoskeletal soft-tissue tumors represent a rare and complex group of heterogeneous lesions. Prompt diagnosis of these uncommon lesions can improve the outcome and increase the patient survival rate. A biopsy examination of soft-tissue tumors with imaging modalities is necessary in all cases of aggressive or undetermined lesions. Although fine needle aspiration can be helpful for the biopsy of certain tumor types, core needle biopsy is a standard procedure in most tertiary sarcoma centers. It has a high diagnostic accuracy, low complication rate and lower price in comparison to open biopsy, and can replace it in the majority of cases of soft-tissue tumor assessment. However, the examining physician has to be familiar with the technique, and the strengths and potential difficulties in performing ultrasound-guided biopsy, as well as possible solutions to obstacles. Item type:Publication, From aneurysmal bone cyst to telangiectatic osteosarcoma with metastasis in inguinal lymph nodes: Case report(National Library of Serbia, 2015); ; ; ; Introduction. Aneurysmal bone cyst is a benign bone lesion composed of blood filled cystic cavities lined by fibrous septa. Its malignant transformation of is a rare event. Case report. We report a case of a lesion in the second metatarsal bone in a 29-year-old male, presented as a slight swelling of the right foot. After the curettage had been done, the diagnosis of aneurysmal bone cyst was made but the recurrence occurred 4 years later. The biopsy of the recurrent tumor showed compact neoplastic tissue consistent with diagnosis of giant cell tumor with malignancy. The malignant component was recognized as a high grade sarcoma with osteoid production. A tumor mass with the whole II metatarsal bone was extirpated and a resected part of fibula was transplanted. A year later, another recurrence occurred, an amputation was performed and a teleangiectatic osteosarcoma with ingvinal lymph nodes metastases was diagnosed. No other tumor mass was confirmed, either clinically or by imaging techniques at the time of his third operation. He died 4 months later with multiple pulmonary metastases. Conclusion. We emphasize the importance of team work in order to achieve the accurate diagnosis, highlighting careful radiological examinations, good sampling and awareness of unusual cases in bone tumor pathology. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, PERCUTANEOUS AS A MINIMALLY INVASIVE TECHNIQUE FOR ACHILLES TENDON RUPTURE(University Ss. Cyril and Methodius in Skopje, 2022) ;Gjorgjieska, Kornelija; ;Dalipi, Rezeart; Trajanovski, AleksandarThe Achilles tendon is the strongest tendon in the human body. When the applied force exceeds the tensile capacity of this tendon, which occurs when the ankle bends abruptly under the action of a compressive force, the tendon is at risk of rupture. The incidence of rupture ranges from 18 per 100,000. Certain antibiotics, long-term steroid use, tendinopathy, and other degenerative disorders are known to increase the risk of rupture, yet the leading cause remains traumatic injury, with predominance in men aged 30 to 50 years. There are two types of treatment: conservative and surgical. Recent studies have shown a difference in outcome between non-surgical and surgical treatment. Meta-analyses have shown that non-surgical treatment increases the risk of re-rupture, while open surgery has a risk of early complications and infection. It is necessary to find a solution to minimize the postoperative complications of open surgery, and possible relapses after the application of conservative treatment, treated with plaster immobilization. Therefore, there is a need for the use of minimally invasive surgery, which has been adopted as a smart way to reduce the rate of postoperative infections. Item type:Publication, DEMOGRAPHYC AND MORPHOLOGIC CHARACTERISTICS OF BONE METASTASES - 11 YEARS SINGLE CENTER EXPERIENCE(Macedonian Association of Anatomists, 2021); ; ; ; Pandilov, StefanThis is an epidemiological study of bone metastasis (BM) diagnosed and treated in a period of 11 years. Age and sex distribution of BM, localization, origin of primary malignancy, plain radiography appearance, therapeutic options and survival of the patients were evaluated. Of 86 patients, 52 were female and 34 male, ranging in age from 44 to 87 years. The most common site of BM was femur, followed by spine and humerus, and the most common primary malignances were in the breast, lung, kidney and prostate, respectively. Almost one third of patients were with unknown primary malignancy at the time of BM diagnosis.The analysis of patients’ plain radiographies revealed that most BM were osteolytic lesions. Bisphosphonate therapy was applied in only 41.86% of patients, radiotherapy was applied in only 2.33% of patients and combination of bisphosphonates and radiotherapy received 8.14% of patients.The rest of the patients, 47.67%, were treated with symptomatic and/or with systemic therapy according to their primary malignancy.The survival analysis did not show statistically significant influence of any of the analyzed parameters in this study on the survival of the patients. Only 26.74% of patients survived 24 months and the high 73.26% survived 1 to 24 months.
