Item type:Publication, Heterotopic pregnancy(JCMICR, 2022-05); ; Heterotopic pregnancy is defined as a condition when intrauterine and extra uterine pregnancy occur simultaneously. It is a life-threatening condition that requires immediate and accurate diagnostics and treatment. It is difficult to diagnose and easily missed. The incidence in the general population is estimated to be 1 in 30,000. With the advent of Assisted Reproduction Techniques (ART) and ovulation induction, the overall incidence of heterotopic pregnancy has risen to approximately 1 in 3,900 pregnancies. Other risk factors include a history of pelvic inflammatory disease (PID), tubal damage, pelvic surgery, uterine Mullerian abnormalities, and prior tubal surgery. Heterotopic pregnancy is a potentially fatal condition, rarely occurring in natural conception cycles. Most commonly, heterotopic pregnancy is diagnosed at the time of rupture when surgical management is required. We present a case of a 30-year-old secondi gravida with history of one spontaneous abortion and pelvic inflammatory disease, who conceived spontaneously and at her fifth week of amenorrhea, was presented to the emergency department with acute pain in lower abdomen, moderate vaginal bleeding and weakness. Laboratory tests and transvaginal ultrasonography revealed the diagnosis of heterotopic pregnancy. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Struma Ovarii: A Rare Ovarian Teratoma and Follicular Adenoma of Thyroid Gland(Austin Publishing Group, 2022); ; ; ; Stojanovska, MIOvarian struma is a germ cell tumor in which thyroid tissue represents more than a half of the tumor and has incidence of 0,3-1% of all ovarian tumors. Most of the patients are asymptomatic and tumor is found incidentally during ultrasound, or with nonspecific symptoms, like pain or abdominal swelling. We report a case of 49-years old woman with no symptoms and an ovarian mass, which was incidentally found during a routine ultrasound examination. On ultrasound she had a tumorous mass on the left ovary with central cystic part filled with dense fluid. Color Doppler showed resistance index RI - 0.64, which did not indicate malignant nature of the tumor, and CA-125 was slightly elevated (38,4 U/ml). The result of the scoring system ROMI was 11 ponts, i.e. low risk for ovarian carcinoma. The patient underwent hysterectomy and bilateral adnexectomy. The final histopathologic report was struma ovarii with well-differentiated neoplasm of uncertain malignant potential. - Some of the metrics are blocked by yourconsent settings

Item type:Publication, Postpartum Juvenile Granulosa Cell Tumor: A Case Report(Austin Publishing Group, 2023-01-25); ; ; ; Akiku Kamberi, ElmedinaGranulosa Cell Tumors (GCTs) are extremely rare, sex cord-stromal tumors constituting only 1% to 2% of all ovarian malignancies. On the basis of age of onset and pathohistological characteristics, these tumors are subdivided into two distinct forms, the adult type (AGCT) and the juvenile type (JGCT), representing 95% and 5% of the tumors, respectively. Compared to the adult type, which is more common in the fifth decade, JGCT is rarely seen and the majority (90%) is reported in prepubertal individuals or those aged less than 30 years. We report an interesting case of a 24-years old woman with enormous ovarian juvenile granulosa cell tumor of the right ovary. Considering all the anamnestic data and ultrasound reports before and during the first pregnancy, where there has been no evidence for the presence of any tumor, we came to conclusion that the tumor grew rapidly during the first year since delivery. Maybe the tumor was present during the pregnancy or even before, but most probably it was not noticed because it small initial size or it was masked by the growing uterus at ultrasound. Item type:Publication, Kombinacija mifepristona i misoprostola u izvođenju medikamentoznog abortusa u prvom trimestru trudnoće(2021); ; ; ; APSTRAKT Cilj. Cilj je bio da se utvrdi efikasnost metode i zadovoljstvo pacijentkinja nakon izvođenja medikamentoznog abortusa radi prekida trudnoće u prvom trimestru. Metode. U retrospektivnoj analizi predstavljamo iskustva u izvođenju medikamentoznog abortusa na našoj klinici u periodu od 1. 10. 2020 do 1. 10. 2021 godine. Tablete mifepristona i tablete misoprostola ordinirane su po šemi prilagođenoj protokolu Svetske zdravstvene organizacije. Rezultati. U periodu od godinu dana na našoj klinici tretirali smo 189 pacijentkinja kombinacijom mifepristona i misoprostola sa ciljem prekida trudnoće u prvom trimestru. Instrumentalna revizija zbog rezidualnih masa u materici nakon dve nedelje urađena je kod osam pacijentkinja (4,23%) dok je procedura ponovljena sa uspehom kod dve pacijentkinje (1,06%). Efikasnost metode bila je 95,8%. Imali smo mali broj komplikacija (produženo genitalno krvarenje, tranzitornu febrilnost, povraćanje i sl.). Primenjenom metodom bilo je zadovoljno 91,4% ispitanica. Zaključak. Medikamentozan abortus u prvom trimestru trudnoće je bezbedna, efikasna metoda i posebno korisna zbog pandemije kovida 19, jer se može primeniti bez hospitalizacije u ambulantnoj sredini, uz konsultaciju s ordinirajućim ginekologom. - Some of the metrics are blocked by yourconsent settings

Item type:Publication, Anastrozole is not Always Successful in Preventing Endometrial Hyperplasia in Patients with Estrogen (+) Breast Cancer – Case Report(Austin Publishing Group, 2023); ; ;Islami, NS ;Sehfulahi, SShabani, ABreast cancer is the most common malignancy among women. Anastrozole is the recommended therapeutic for long-term postoperative therapy in postmenopausal women with estrogen (+) breast cancer. Its advantage over Tamoxifen lies in the fact that it does not have a stimulating effect on the proliferation of the endometrium, and therefore does not represent a risk for the development of endometrial atypia and endometrial cancer. However, it is not always 100% effective in suppressing endometrial proliferation and anaplasia. Our case shows exactly the same situation. In our case, endometrial atypia was diagnosed despite Anastrazole therapy for estrogen (+) breast cancer. The lesson that can be drawn from our case is that cases that are on long-term hormonal therapy with Anastrazole due to previous estrogen (+) breast cancer should be vigilantly monitored for the possible development of premalignant and malignant endometrial changes, including ultrasound gynecological examination at least once/6 months and, if necessary, performance of fractional exploratory curettage. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Case of an Low Grade Appendical Mucinous Neoplasia (LAMNS) Presenting as a Right Ovarian Mass(Austin Publishing Group, 2022); ; ; ; Appendiceal carcinoma has an incidence of 0.12 cases per 1,000,000 people per year [1]. Of all intestinal tumors, malignant tumors of appendix are less than 0.5%. They are classified into three groups: carcinoid tumors, mucinous cystadenocarcinomas and adenocarcinomas, based on architectural and cytologic features [2]. Appendiceal carcinoma present with acute right abdominal pain, and can also present as a palpable abdominal mass, acute intestinal obstruction or ascites. Most of them are diagnosed unexpectedly from histopathologycal analysis after a simple appendectomy. That is why it is important for gynecologists and general surgeons to consider appendiceal mucinous cystadenomas in their differential diagnosis for a pelvic mass, because early recognition can prevent iatrogenic rupture and subsequent spilling of potentially malignant mucin-producing cells in a peritoneal cavity. Item type:Publication, FIRST AND SECOND TRIMESTER MEDICAL ABORTION - PILOT STUDY IN REPUBLIC OF NORTH MACEDONIA(Македонско лекарско друштво = Macedonian medical association, 2021); ; ; ; Introduction. Medical abortion is a procedure in which medication is used to end a pregnancy up to 22 weeks of gestation. The aim of this study was to assess the efficiency, safety and acceptance of medical abortion in the first and second trimester using a regimen of medications, mifepristone followed by misoprostol. Methods. In a prospective study, conducted at the University Clinic for Gynecology and Obstetrics in Skopje in the period from March to November 2021, women that came for medical abortion in the first and second trimester were enrolled. They were divided into two groups: Group I (5 to 12 weeks of gestation) and Group II (12 to 22 weeks of gestation). Participants in Group I were given 200 mg mifepristonе for peroral application and after 24 hours instructed for sublingual application of 800 µg misoprostol at home. Participants in Group II were hospitalized and treated with the same regimen. Additional dosage of misoprostol was given to complete abortion. Results. A total of 208 women with medical abortion up to 22 weeks of gestation (173 in the first group and 35 in the second group) were enrolled in the study. In the first group a complete uterine evacuation was achieved in 168 women (97.1%) and in 25 women (71.4%) in the second group. Acceptability of the method was high among both groups (95.14 % in Group I and 82.14 % in Group II). All doctors/clinicians who participated in this pilot study were satisfied with the method. Conclusions. Medical abortion is effective, safe and acceptable option for women in the first and second trimester. Acceptability of the method was high among both groups.
