Item type:Publication, Sentinel lymph node status versus tumor characteristics, neutrophil to lymphocyte ratio, C-reactive protein levels and C-reactive protein to albumin ratio-prognostic factors for primary cutaneus melanoma(Springer Science and Business Media LLC, 2019-09-18); ; ; Aim/Introduction: The aim of this study was to identify tumor characteristics of primary malignant melanoma, neutrophil to lymphocyte ratio (NLR), C - reactive protein (CRP) levels and C - reactive protein to albumin ratio (CRP/Alb) predictive of sentinel lymph node (SLN) positive status and their impact on disease recurrence, melanoma specific survival (MSS), disease free survival (DFS) and overall survival (OS) period. Materials and Methods: 100 patients with primary malignant melanoma, clinically staged T1b/T2+ (1-4mm thick) N0, M0 underwent SLN detection procedure. Tumor characteristics, NLR, CRP levels and CRP/Alb ratio were analyzed in correlation with SLN status. The optimal cutoff values of NLR and CRP/Alb were determined by the receive operative characteristics (ROC) analysis. Univariate and multivariate Cox regression analyses, t - test for continuous or X2 test for categorical variables and Kaplan-Meier curve analyses were performed to assess factors predicting SLN positive status, regional recurrence rate, DFS, MSS and OS period. Results: We identified Breslow`s thickness (OR=1,51; 95% CI 1,02- 2,01; p=0,034), presence of ulceration (p=0,041), lymphocytic infiltration (OR=0,42; 95% CI 0,18-0,99; p=0,02), high mitotic rate (p=0,031), high Ki67 index (p<0,01), high NLR (HR=2,45; 95% CI 1,52-3,34; p<0,01), increased CRP levels (HR=1,53; 95% CI 1,33-1,75; p<0,001) and high CRP/Alb ratio (HR=2,01; 95% CI 1,15-3,52; p=0,016) as independent predictors of SLN positive status. SLN status was the strongest predictor (HR=0,241; 95% CI 0,45-1,01; p=0,04) of DFS, MSS and OS period. Conclusion: We confirmed Breslow’s thickness, presence of ulceration, lymphocytic infiltration extent, high mitotic rate, high Ki67 index, high NLR, increased CRP levels and high CRP/Alb ratio as factors predictive of SLN metastatic involvement. We also confirmed SLN status to be the most significant independent predictor of DFS, MSS, OS and risk of regional recurrence. Item type:Publication, Coexistence of Paget disease and lung cancer - importance of patient management(Discover STM Publishing Ltd., 2022); ; ;Stefanovski Todorova, Dusica; Background: The coexistence of metabolic bone disease, such as Paget disease (PD) and malignant tumor is frequently described in patients with breast, prostate, lung, lymphoproliferative, colorectal, and renal neoplasms. Indeed, when evaluating cancer patients, there may be diagnostic dilemma of interpreting some changes in bone involvement, whether they are malignant or metabolic changes, since some radiological features of metabolic bone disease can mimic those of bone metastases. Case Presentation: We report a case of 55-year-old male patient, with diagnosed lung adenocarcinoma, referred to the Nuclear Medicine Department for a bone scintigraphy (BS), for evaluation of presented bone metastasis. The Single photon emission computed tomography (SPECT)/computer tomography (CT) scan revealed intensive metabolic accumulation of the radiotracer in cervical vertebra C7, and thoracic vertebra Th1, Th6, Th11, Th12, as well as lumbar vertebra L4, L5, left sacroiliac joint, both iliac and pubic bones and left proximal femur. The FDG PET/CT showed diffuse, but mildly increased uptake in Тh6, Тh11, L4, L5 and pelvic bones (SUV max = 3.3). The CT revealed destruction of Th11 body with compression fracture. Diagnosis of PD was made based on the findings of BS and PET/CT and further confirmed on bone biopsy and the patient was started with bisphosphonate therapy. Conclusion: BS and SPECT/CT are of great importance in accurate detection of incidental PD in patients with lung cancer. - Some of the metrics are blocked by yourconsent settings

Item type:Publication, Nuclear medicine hybrid imaging (SPECT/CT) in distinguishing the presence of a hepatic hemangioma: Single center study(Centre for Evaluation in Education and Science (CEON/CEES), 2023); ; ;Stoilovska-Rizova, Bojana; <jats:p>Objective. Hepatic hemangioma (HH) is the most common benign liver tumor, and the second most frequent tumor in the liver after hepatic metastasis. The SPECT/CT hybrid technique will be beneficial for the investigation of this type of HH since it can precisely identify the hepatic lesions. The aim of this study was to reevaluate the sensitivity and specificity of the nuclear medicine method for confirmation or exclusion of benign hemangioma of the liver based on a series of cases at our department and briefly review the literature. Methods. We retrospectively analyzed 107 patients, 62 females (57.94%) and 45 males (42.05%) with mean age 50.05±11.92 years, referred to the Nuclear Medicine Department for 99mTc-RBC scintigraphy of the liver to conclude or exclude the presence of HH, in the period 2019 to 2020. Results. Hepatic scintigraphy located the hemangiomas mostly in the right hepatic lobe. The size of the lesion varied from to 6-140 mm (46.04 ± 27.1); 13 hemangiomas were described as giant. SPECT-CT confirmed HH in 1 patient that was negative on ultrasound, besides 30/87 (34.48%) patients who were described as positive on US and turned out to be negative on the scintigraphic method. In 53 (60.92%) patients, positive matching of the US images and hybrid SPECT/CT imaging for HH was found. Most of the patients had benign referral diagnosis, while 12 of them had confirmed malignant diagnosis in whom eight (n=8, 66.67%) were confirmed free of hepatic metastasis and SPECT-CT detected HH, while in 4 patients who were described as positive for HH on the CT scan, the scintigraphic method excluded HH and further evaluation of the hepatic lesion was needed. Conclusion. Hepatic hemangiomas require a careful diagnosis to differentiate from other focal hepatic lesions, cooccurring diagnoses are also possible. Differentiating between HH and hepatic metastatic disease is a typical clinical difficulty when the problem is present in staging or monitoring patients with oncological disease.</jats:p> - Some of the metrics are blocked by yourconsent settings

Item type:Publication, PRIMARY HYPERPARATHYROIDISM INDUCED BY AN ECTOPIC ADENOMA, INITIALLY PRESENTED AS BROWN TUMOR OF THE TIBIA(Department of Anaesthesia and reanimation, Faculty of Medicine, Ss Cyril and Methodious University Skopje, Macedonia, 2020-10); ; ;Samardziski, Milan; The classic manifestation of unchecked, advanced hyperparathyroidism is the brown tumor, a non-neoplastic giant cell osteomedullary lesion. We report a case of, these days rarely seen manifestation of advanced hyperparathyroidism, brown tumor due to an ectopic mediastinal parathyroid adenoma. A 63-years-old woman presented with an expansile osteolytic lesion of the right tibia on the radiographic images. Because of the suspicion of primary or secondary bone tumor, bone scintigraphy was performed. The bone scan was indicative of metabolic bone disease. Furthermore, laboratory investigations were obtained that revealed hypercalcemia, hypophosphatemia, increased level of alkaline phosphatase, and parathyroid hormone. Subsequently, ultrasound of the neck was performed, but no enlarged parathyroid glands were detected, so the patient underwent dual-phase 99mTc-methoxy-isobu- tyl-isonitrile (MIBI) parathyroid scintigraphy using a hybrid SPECT/CT gamma camera. The MIBI scan showed ectopic hypermetabolic parathyroid tissue in the anterior mediastinum, as well as an additional appearance of focal radiotracer uptake in the sternal end of the right clav- icle, suggesting the presence of a brown tumor. After thorough clinical workup, the diagnosis was in favor of primary hyperparathyroidism and the patient underwent surgical resection of the ectopic parathyroid gland with gamma-probe guidance, later histopathologically confirmed it to be a parathyroid adenoma. Postoperatively her PTH level dropped and the electrolyte status normalized within 6 months. Osteolytic brown tumors can easily imitate bone malignancy and should be evaluated with caution. The use of intraoperative gamma-probe guidance could support the complete removal of the parathyroid adenoma tissue ensuring the surgical and therapeutic success for the patient.
