Item type:Publication, Synchronous occurrence of ileal stromal tumor (GIST) and colonic adenocarcinoma: a case report.(2015) ;Trajkovska, Elizabeta; ; ; Introduction: Gastrointestinal stromal tumors (GIST) are the most common mesenchymal tumors of the digestive tract. There is an increasing number of literature reports on synchronous occurrence of gastrointestinal stromal tumors and another malignancy of distinct etiology and evolution. The most reported cases include gastric synchronous occurrence of gastrointestinal stromal tumors and adenocarcinoma and gastric gastrointestinal stromal tumors and colonic adenocarcinoma. Case report: We present a case of a 77-old female, with synchronous cecal moderately differentiated adenocarcinoma in Stage IIA according to the TNM classification and ileal spindle cell type GIST with low malignant potential, positive for c-Kit, CD34, vimentin, Actin, and negative for S100. Conclusion: The synchronous occurrence of small bowel gastrointestinal stromal tumors and other primary gastrointestinal malignancies has been rarely reported. There is a need of further investigations to identify the relationship between gastrointestinal stromal tumors and colorectal cancers. - Some of the metrics are blocked by yourconsent settings

Item type:Publication, Predictive Value of Alvarado, Acute Inflammatory Response, Tzanakis and RIPASA Scores in the Diagnosis of Acute Appendicitis(Vilnius University Press, 2020-12-29); ; ; ;Markov, PetarMladenovik, Dragoslav<jats:p>Introduction. The diagnosis of acute appendicitis (AA), as the most common cause of acute abdominal pain, has changed in the past decade by introducing scoring systems in addition to the use of clinical, laboratory parameters, and radiological examinations. This study aimed to assess the significance of the four scoring systems (Alvarado, Appendicitis Inflammatory Response (AIR), Raya Isteri Pengiran Anak Saleha Appendicitis (RIPASA) and Tzanakis) in the prediction of delayed appendectomy. Materials and methods. The study included 100 respondents, who were diagnosed with AA in the period from January 2018 to February 2019 and were also operated on. In addition to the clinical, laboratory, and ultrasonographic examinations, four scoring systems (Alvarado, AIR, RIPASA, and Tzanakis) were used to diagnose AA. According to the obtained histopathological (HP) findings, the patients were divided into 3 groups: timely appendectomy, delayed appendectomy and unnecessary appendectomy. Using the sensitivity and specificity of all 4 scoring systems, ROC analysis was performed to predict delayed appendectomy. Results. In the study that included 100 patients (58% men, 42% women), after the appendectomy was performed, the resulting HP showed that 74% had a timely appendectomy, while 16% had delayed and 10% had an unnecessary appendectomy. For the prediction of delayed appendectomy, the area under the ROC curve showed a value of 0.577 for the Alvarado score, 0.504 for the AIR, 0.651 for the RIPASA, and 0.696 for the Tzanakis. Sensitivity and specificity for the Alvarado score was 54% and 62%, for RIPASA 62.5% and 63.5%, for Tzanakis 69% and 60.8%, respectively. Combining the three scoring systems (Alvarado, RIPASA, and Tzanakis), the surface area under the ROC curve was 0.762 (95% CI 0.521–0.783), with a sensitivity of 85% and a specificity of 62%. Conclusion. In our study, the diagnostic accuracy of RIPASA and Tzanakis showed better results than Alvarado, while AIR cannot be used to predict delayed appendectomy in our population. However, the simultaneous application of all three scoring systems, RIPASA, Tzanakis and Alvarado, has shown much better discriminatory ability, with higher sensitivity and specificity, as opposed to their use alone. Combining scoring systems should help in proper diagnosis to avoid negative appendectomy, but additional studies with a larger number of patients are needed to support these results.</jats:p> - Some of the metrics are blocked by yourconsent settings

Item type:Publication, Synovial sarcoma of the liver - a case report(Scientific Foundation SPIROSKI, 2011-06-15); ;Filipovski, Vanja ;Banev, Saso; Jovcevski, AlenWe report a case of synovial sarcoma of liver in a 44 year old man, presented as a tumor mass in left hepatic lobe. The patient was admitted at the hospital with clinical symptoms of acute abdomen and severe pain in the right upper quadrant. Imaging examinations showed a tumor mass in the left hepatic lobe and free liquid in the abdominal cavity, due to the rupture of the tumor. A resection of 2 segments of the left hepatic lobe, where the tumor was located, was performed. Morphological, immunohistochemical and FISH studies confirmed the diagnosis of monophasic synovial sarcoma. Additional clinical and imaging examinations, made after the surgery, did not confirm tumor mass in any other localization. The patient refused any therapy other than surgery, at that time. A relapsing tumor mass was found 6 months later and another surgical intervention was done. The patient received five monotherapy cycles of Doxorubicin, 75 mg/m2, after the second surgical intervention. He is still alive 11 months after the first operation receiving the same therapy and having second relapsing inoperable tumor mass filling the retroperitoneal space and a great fraction of the abdominal cavity. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Serum Matrix Metalloproteinase-2, -7 and -9 (MMP-2, MMP-7, MMP-9) levels as Prognostic Markers in Patients with Colorectal Cancer(University of Sarajevo Faculty of Health Sciences, 2012-12-15); ; ; ; <jats:p>Introduction: Matrix metalloproteinases are produced by tumour cells, hence, they may be associated with tumour progression including invasion, migration, angiogenesis and metastasis. Finding prognostic markers to better identify patients with higher risk for poor survival would be valuable in order to customize pre- and postoperative treatment as well as to enable closer follow-up of these patients. Aim of our study was to examineMMP-2, MMP-7 and MMP-9 serum levels and correlated them with pathological data such as stage of the colorectal cancer (CRC) and outcome.Methods: The investigation included 82 patients with operable CRC without distant metastases, who had underwent blood tests in order to determine the MMP-2, MMP-7 and MMP-9 serum levels in the following time periods: preoperatively, 3, 6, 9 and 12 months postoperatively.Results: The values of the investigated MMPs decrease postoperatively and start to increase 6 month later in patients of all stages of the disease, reaching the highest value 12 month postoperatively with statistically important differences of MMP-2, MMP-7 and MMP-7 serum levels in terms of disease staging and defined points of time. Analysis of the results showed that the MMP-2 serum levels obtained 3 and 12 months postoperatively,than MMP-7 serum levels 12 months postoperatively and the MMP-9 serum levels in all analyzed points in time were in significant association with the CRC patients’outcome.Conclusion: The MMP-2, MMP-7 and especially MMP-9 serum values could be important indicators for diagnosis of the patients with CRC and for monitoring of disease progression.</jats:p> - Some of the metrics are blocked by yourconsent settings
Item type:Publication, THE NEW CLASSIFICATION OF DISTAL EXTRAHEPATIC CHOLANGIOCARCINOMA-MORPHOLOGICAL ANALYSIS AND REVIEW OF LITERATURE(MACEDONIAN ASSOCIATION OF ANATOMISTS AND MORPHOLOGISTS, 2011); ; ; ; Extrahepatic cholangiocarcinoma (EH-CCa) is a malignant tumor that arises from the ductal epithelium of the extrahepatic bile duct and it has been divided into perihilar and distal at the level of the cystic duct . As relatively rare disease perihilar and distal extrahepatic cholangiocarcinoma should be viewed as independent entities because of their distinct biology and management. The aim of the study was to make morphologic analysis of archive material of diagnosed EH-CCa at the Institute of pathology, Medical Faculty, Skopje in 5-year period and revision of the cases according to 7th edition (AJCC/UICC) TNM staging classification. We used archive material from the data base of the Institute of pathology, Medical Faculty, Skopje, for 7 cases diagnosed as distal EH-CCA between year 2006 and year 2010. Four analyzed adenocarcinomas showed periductal-infiltrating pattern of growth and 3 cases had massforming characteristics. One case revealed pancreatic involvement, 5 infiltrated the periductal soft tissue and 1 was confined to the ductal wall. In 3 cases peri/intraneural invasion was found and 2 cases had vascular tumor embolusses. There were 2 lymph nodes with metastases from 10 examined in only one case, in two cases there was no surgical resection of lymph nodes, and the rest cases were free from metastatic disease. Positive surgical margines we found in 2 cases and 2 were negative. The most of EH-CCA were diagnosed in stage IB, one was in stage IA and one in stage IIB. The purpose of 7th edition AJCC/UICC TNM staging system is predominantly prognostic and will require additional validation. - Some of the metrics are blocked by yourconsent settings
