Item type:Publication, SERTOLI-LEYDIG CELL TUMOR OF THE OVARY AS AN INCIDENTAL FINDING IN A PATIENT UNDERGOING HYSTERECTOMY DUE TO RECURRENT ABNORMAL UTERINE BLEEDING(Faculty of Medicine, Ss. Cyril and Methodius University in Skopje, 2023) ;Stojchevski, Sasho; ; ; Sertoli-Leydig cell tumor (SLCT) of the ovary is a rare neoplasm that accounts for only 0.5% of all primary ovarian neoplasms. Clinical and histopathological presentations of the tumor are different, which makes diagnosis and treatment difficult. We present a case of a 67-year-old female patient, who presented at the University Clinic for Gynecology and Obstetrics with abnormal uterine bleeding. Explorative curettage was performed for the second time after 1 year, with a histopathological finding of endometrial hyperplasia. The patient underwent an excision of a lung tumor 2 years ago, with a histopathological finding of synovial sarcoma of the lungs, after which she received adjuvant chemotherapy. On transvaginal ultrasonography, the patient had no suspicious pathological findings on the uterus and ovaries. Laboratory tests of tumor markers did not show any elevation. Due to recurrent abnormal bleeding from the uterus, the patient underwent an abdominal hysterectomy with bilateral adnexectomy. Histopathology: Sertoli-Leydig tumor of the right ovary with retiform histoarchitectonics, stage IA and atypical endometrial hyperplasia. Sertoli-Leydig cell tumors have a relatively good prognosis, but their management, surgical treatment and postoperative follow-up due to their rarity is a challenge even today. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, MENAGEMENT OF BUTTONHOLE RECTAL INJURY AFTER VAGINAL DELEVERY - A CASE REPORT(Македонско лекарско друштво = Macedonian Medical Association, 2020); ; ; ; Abstract Introduction. A special form of higher degree perinealtear is a laceration of the anorectal epithelium with in-tact external anal sphincter muscle (“buttonhole tear”). This is very rare but, when not treated, carries the risk of a rectovaginalfistula. Case Report. We present a case of a nullipara, in which a spontaneous vaginal delivery of a fetus in the vertex presentation was complicated by a buttonhole rectal tear with a partial lesion of the sphincters. It was recognized on the 4thpostpartum day, and a recon-struction of the lesion with a diversing colostomy was made soon. Upon returning the colostoma, the patient at 6 months postpartum has good continence and is in a good condition. Discussion. There are very few cases reporting an isolated rectal lesion during parturition. Several factorsmay play a role in the etiology of these lesions, inclu-ding instrumentation, birth weight of more than 4 kilo-grams, midline episiotomy, persistent occipitoposterior presentation, nulliparity, tissue factors, and second sta-ge >1 hour. Obstetric anal sphincter and rectal injuriescan be missed if rectal examination is not carried out asa standard procedure prior to suturing. This can have a devastating effect on the physical and emotional well-being of women.Conclusion. Careful examination of the vagina and the rectum should be performed in all cases of perineal tears following a vaginal delivery. Buttonhole injuries, although rare, should be considered as severe traumas similar to the 4th degree lacerations and managed prom-ptlyby experienced surgeons. - Some of the metrics are blocked by yourconsent settings

Item type:Publication, Korelacioni mes rashësisë së endometriumit dhe qasjes patohistolog jike te pacientet në postemnopauzë(SHMSHM / AAMD, 2017-10); Aim of the study was to trial to investigate the validity of the detected ultrasonic thickness of the endometrium in the prediction of endometrial cancer in postmenopausal patients with and without uterine bleeding, confirmed by histopathological analysis of material obtained by exploratory curettage of the endometrium. Material and Methods: This study included 120 patients recruited in post-menopausal age of the University Clinic of Gynecology and Obstetrics - Skopje, who were divided into two groups. Test group included 80 patients with postmenopausal detected ultrasonic endometrial thickness greater than or equal to 5mm. They were divided into two subgroups - subgroup A: Patients with uterine bleeding (40 patients) and subgroup B: patients without uterine bleeding (40 patients), which were divided based on ultrasound endometrial thickness of endometrium of 5-8mm; from 8-11mm; more than 11mm. The control group included 40 postmenopausal patients hospitalised because urogynecological pathology with previous ultrasound detected endometrial thickness of less than 5mm and orderly histopathological findings derived from material in exploratory curettage of endometrium. In each patient detailed history was taken and curettage exploratory fractionated and material was send to histopathological analysis. Results. The thickness of the endometrium is endometrial significant predictor of malignancy (p <0,05). With each millimeter increase in endometrium significantly increases the likelihood of endometrial malignancy of 1,178 [p = 0,002, 95% CI = 1,065-1,304] times. Conclusion: In postmenopausal patients likelihood of endometrial cancer significantly increases by 1,178 times. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, ROBSON CLASSIFICATION OF CESAREAN SECTION IN NORTH MACEDONIA - CURRENT TRENDS(Македонско лекарско друштво = Macedonian medical association, 2021); ; ; ; Dalipi, AdelinaAbstract Introduction. Over the last few decades, the global cesarean section rate has significantly increased and reached an unprecedented level. The World Health Organization (WHO) has advised that cesarean section (CS) rates should not rise above 15%. Several classify¬cation systems have been proposed to tackle the increased cesarean section epidemic. Most of the countries have adopted and started using the Robson (10 groups) classification as the best and the one that is the easiest internationally applicable CS classification. Aim. To present the Robson classification as a way to start better classification of cesarean section and hence to reduce the number of unnecessary cesarean section deliveries. Methods: This study was realized at the University Clinic for Gynecology and Obstetrics in Skopje, North Macedonia. It is a retrospective study where two years were compared. Results. The rate of cesarean sections for 2017 was 38.5% and for 2019 42.6%. Categorization of deliveries according to Robson criteria showed a different rate of cesarean section for each subgroup. Discussion. The implementation of the Robson classi-fication in most countries has shown a reduction in the number of cesarean deliveries and thus a reduction in overall maternal and neonatal morbidity and mortality. The analysis has shown that group 5 had the largest number of cesarean section deliveries in both years, 2017 and 2019; these were patients with previous cesarean sections. They were followed by group 1 and 2, or pri¬mi¬para with spontaneous onset and induced delivery. Conclusion. The goal of Robson clasification is to identify the target groups that contribute most in the percentage of cesarean sections and to act on these tar-get groups through appropriate education and training. - Some of the metrics are blocked by yourconsent settings
Item type:Publication, Adnexal masses in pregnancy(Македонското Лекарско Друштво во соработка со Медицинскиот Факултет - Скопје, 2023) ;Dalipi, Adelina; ; Objectives: The objective of this case is to present a rare case of maternal ovarian mucinous cystadenocarcinoma during pregnancy and its outcome. Only 0.05-2.4% of pregnancies are complicated by an adnexal mass, and approximately 1-6% of these masses are malignant. The majority of ovarian masses are diagnosed by a routine ultrasound examination or cesarean delivery. Adnexal masses less than 5cm usually are benign and in 70% of cases spontaneously resolve by the second trimester while the adnexal masses 5 cm or greater are dermoids and 10% found in pregnancy are malignant. One-half of all ovarian malignancies in pregnant women are epithelial ovarian tumors. Material and Methods: We present a case of 27-year-old pregnant women who was incidentally diagnosed by ultrasound examination with a large multilocular cystic adnexal mass in right ovarium at the first trimester (ml III) with diameter 88x85mm which enlarged to 88x140mm at third trimester (ml X). In week 40 of pregnancy, the patient gave birth to a healthy baby through cesarean section, during that was performed and right salpingo-oophorectomy, Results: The pregnancy was terminated at term with good pregnancy outcome. The histopathology report of right salpingo-oophorectomy describe mucinous cystadenocarcinoma of the right ovarium Stage IA according to FIGO classification. Item type:Publication, OBJECTIVE METHODS FOR ASSESSMENT OF DEEP MYOMETRIAL AND CERVICAL INVASION IN PATIENTS WITH ENDOMETRIAL CANCER(SHMSHM - AAMD, 2023-12); ; ;Stojchevski, Sasho; Introduction: Endometrial cancer is the most common gynaecological malignancy. This study analyzed different objective methods with transvaginal ultrasonography for the assessment of deep myometrial and cervical invasion in patients with endometrial cancer to choose the optimal surgical treatment. Material and methods: It represents a prospective cohort study in which 45 patients with a histological diagnosis of endometrial cancer were analyzed. They are examined with transvaginal ultrasound to assess deep myometrial and cervical invasion. Results: Objective parameters such as those of Gordon result in a sensitivity of 76% and a specificity of 85% and Carlson with a sensitivity of 72% and specificity 95%. 3D ultrasonography resulted in a sensitivity of 92% and a specificity of 78%. The objective assessment of cervical tumour invasion below the OUI (internal uterine orificum) had a sensitivity of 70% and a specificity of 85%, and the one with a cut-off <20.5mm distance from the OUE (external uterine orificium) to the lower margin of the tumour had a sensitivity of 86% and specificity 75%. Discussion: The therapeutic modality in patients with endometrial cancer depends on several factors such as histological type, nuclear grade, myometrial and cervical invasion, which, if present, require radical surgical treatment and the need for evaluation of lymph nodes for appropriate surgical staging. Conclusion: The obtained results are comparable with relevant works on this issue. Transvaginal ultrasonography is similarly effective to nuclear magnetic resonance for assessing deep myometrial and cervical invasion. - Some of the metrics are blocked by yourconsent settings

Cyril and Methodius University in Skopje, R.N. Macedonia, 2023-12); ; ;Stojchevski, Sasho; Introduction: Endometrial cancer is the most common gynecological malignancy. In this study, the diagnostic accuracy of subjective methods with transvaginal ultrasonography for the assessment of deep myometrial and cervical invasion in patients with endometrial cancer was analyzed in order to choose the optimal surgical treatment. Material and Methods: It represents a prospective cohort study in which 45 patients with a histological diagnosis of endometrial cancer were analyzed. They are examined with transvaginal ultrasound to assess deep myometrial and cervical invasion with subjective methods based on IETA (International Endometrial Tumor Analysis) terminology. Results: Subjective assessment of deep myometrial invasion based on the International Endometrial Tumor Analysis (IETA) endometrial cancer terminology yielded a sensitivity of 88% and a specificity of 65%. Subjective assessment of deep myometrial invasion includes tumor echogenicity, endometrial-myometrial junction, color Doppler result and vascular pattern.Subjective assessment of cervical invasion includes loss of clear demarcation of tumor tissue from the cervical stroma and in the presence of stronger perfusion. Our subjective assessment of cervical invasion had a sensitivity of 76% and a specificity of 92%. The obtained results are comparable with relevant works on this issue. Conclusion: Transvaginal ultrasonography is similarly effective to nuclear magnetic resonance for assessing deep myometrial and cervical invasion; therefore, it can be used as a preoperative triage diagnostic tool to choose a surgical modality from which the mostly poor surgical candidate patients would benefit.
